<div>We are looking for a Care Coordinator with strong skills to join our Biddeford BHH team. You will work with the children of families within our catchment area based out of the Biddeford office which covers York County. You will join a team of Care Coordinators, family support persons, clinical leads, and medical/psychiatric/and nursing supports. We are looking for someone with the ability to manage a busy schedule, the needs of each family, and documentation needs.
The position is 32 hours per week and pays between $21.00-$27.00 per hour with opportunities for monthly and quarterly incentives.
DUTIES:
Care Coordinators are the central piece to a network of providers working with each child. You will work to help coordinate and collaborate with each provider working with the child to ensure that their needs are being addressed - home needs, school needs, medical needs, and/or community needs. You will complete intakes for new clients, complete psychosocial assessments and other intake assessments, complete weekly documentation needs, make referrals, and complete initial and 90-day treatment plans. You will then continue to provide a minimum of monthly contact with each family and continue to collaborate with providers.
QUALIFICATIONS:
BA in related field required. Strong organization and communication skills. Need savvy computer skills to manage multiple systems. All applicants must maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving policy.

Children's BHH Care Coordinator/Case Manager

<div>Come join a vibrant, interdisciplinary team serving the complex needs of adults with behavioral health challenges in the greater Augusta area. We are looking for a collaborative, engaging, and driven care coordinator (case manager) who has their MHRT/C or is MHRT/C eligible.
This position pays $21.00 to $26.00 per hour depending on experience.
DUTIES:<ul><li>
Provide intake/assessment when clients are referred and ongoing assessment as required.</li><li>
Facilitate Individual Service Plan team meetings and develop a plan of care (ISP) for the client with input from the team.
</li><li>Oversee implementation of ISP, monitor and document progress.</li><li>
Participate in a team-based approach to care, by working in collaboration with Clinical Team Leader, Peer Partner, Nurse Care Manager, and Psychiatric and Medical Consultants.</li><li>
Maintain regular contact with all team members and other community professionals involved with the client.</li><li>
Provide support through phone contact and face-to-face meetings through home visits and community outreach. Home visiting required, as well as the ability to work outside traditional work hours to meet clients scheduling needs.</li></ul>
QUALIFICATIONS:
Associates degree is preferred. Experience in the social services field preferred. MHRT/C or MHRT/C eligible. Must maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving Policy.
