RN Case Manager
Position Overview
The Registered Nurse Case Manager (RNCM) is experienced in nursing, and leads each patient&rsquo;s hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff. The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.
Essential Functions and Skills 
Patient Care<ul><li>
Provides care in accordance with the Nurse Practice Act and professional nursing standards.</li><li>
Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history.</li><li>
Develops, implements, and revises the Plan of Care based on ongoing patient evaluations.</li><li>
Administers medications and treatments as prescribed in the physician&rsquo;s plan of care.</li><li>
Educates patients and families on self-care, medications, and diet as appropriate.</li><li>
Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition.</li><li>
Coordinates care with physicians, interdisciplinary teams, and community health partners.</li><li>
Ensures a safe environment for patients and supports their activities of daily living.</li><li>
Provides emotional support and counseling to patients and families.</li><li>
Arrange for necessary medical equipment and supplies.</li><li>
Ensures HIPAA compliance and patient confidentiality.</li></ul>
Additional Duties<ul><li>
Participates in on-call visits as required including evenings and weekends.</li><li>
Attend interdisciplinary group meetings.</li><li>
Supervises ancillary personnel and delegates responsibilities as needed.</li><li>
Engage in professional development through training and continuing education.</li><li>
Contributes to quality assessment and performance improvement activities.</li><li>
Performs other duties as assigned.</li></ul>
Requirements/Qualifications<ul><li>
Must have a current Registered Nurse (RN) license in state(s) of practice.</li><li>
Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.</li><li>
Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.</li><li>
Ability to pass DHS background, consumer report, and drug test.</li><li>
Valid State Driver&rsquo;s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.</li></ul>
Preferred<ul><li>
1+ years of recent experience in hospice/home care preferred.</li><li>
2+ years of Nursing experience, preferred</li></ul>

Triage RN
Position Overview 
The Triage Registered Nurse is an experienced nurse responding to overnight and weekend calls regarding the changing needs of hospice patients. The Triage Registered Nurse is responsible for answering questions, gathering information, assessing patient needs and when needed, coordinating and directing hospice services/care via phone conference. The Triage Registered Nurse is experienced in nursing with an emphasis on palliative care. 
 
Essential Functions and Skills 
Patient Care<ul><li>
Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.</li><li>
Receives calls related to patient care and instructs patient/caregiver on appropriate interventions based on nursing knowledge.</li><li>
Evaluates available options for interventions and uses critical thinking when selecting which intervention is best for the patient and/or caregiver.</li><li>
Determines the need for nursing visit based on EMR and telephone call. Dispatches a visit RN or LPN as appropriate.</li><li>
Completes, submits, and maintains accurate and relevant clinical notes regarding patient&rsquo;s condition and care given. Records pain/symptom management and changes/outcomes as appropriate.</li><li>
Communicates with the physician regarding patient needs and reports changes in patient condition; obtains/receives physician orders as required.</li><li>
Facilitates the patient&rsquo;s efforts toward self-sufficiency and optional comfort care.</li><li>
Counsels and works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.</li><li>
Ensure arrangements for equipment and other necessary items and services are available.</li><li>
Responsible for maintaining compliance with all HIPAA regulations and requirements. Treats all patient information as confidential and utilizes protected health information on a minimum necessary basis.</li></ul>
Additional Duties<ul><li>
Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in- service classes.</li><li>
Actively participates in quality assessment and performance improvement teams and activities as assigned.</li><li>
Other duties as assigned.</li></ul>

The salary range for this role is $74,000-85,000 annually. An employee&rsquo;s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.</div>
