Registered Nurse Case Manager Requirements &amp; Responsibilities:
<ul><li>Associates, Bachelors, or Masters of an accredited nursing program (RN)
</li><li>1+ years of recent acute care experience required; hospice/home health experience preferred
</li><li>Complete initial and ongoing comprehensive assessments to determine hospice needs
</li><li>Develop individualized care plans with the patient and family, updating as needs evolve
</li><li>Administer medications and treatments as prescribed
</li><li>Educate patients and caregivers on care techniques, medications, diet, and symptom management
</li><li>Rotate on-call duties and ensure seamless care transitions
</li><li>Must have reliable transportation, valid driver&rsquo;s license, and proof of auto insurance

As a Registered Nurse On-Call, you are an essential part of our 24/7 care model. You&rsquo;ll provide skilled, responsive care during evenings, nights, and weekends addressing urgent patient needs, completing after-hours admissions, and offering calm, confident guidance during critical moments. The 7 days on / 7 days off overnight schedule offers professional autonomy, work-life balance, and the opportunity to make a profound impact when it is needed most.

Registered Nurse On-Call Requirements &amp; Responsibilities:
<ul><li>Graduate of an accredited nursing program (Associates, Bachelors, or Masters)

</li><li>Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates

</li><li>Administer medications and treatments as prescribed, offering education and support to patients and caregivers
</li><li>Maintain timely and accurate clinical documentation of care, outcomes, and communication
</li><li>Must have reliable transportation, valid driver&rsquo;s license, and proof of auto insurance

The Nurse Practitioner supports patients and families during the final stages of illness, offering thoughtful medical care and a steady, compassionate presence. They help families understand what to expect and guide them in making informed choices that align with the patient&rsquo;s goals and wishes. By managing medications, treatments, and recertifications, Nurse Practitioners provide continuity and stability, allowing families to focus on comfort, dignity, and meaningful time together.

Nurse Practitioner Requirements &amp; Key Responsibilities:<ul><li>
Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice.</li><li>
Degree from an accredited NP program required.</li><li>
Minimum two years of RN experience; preferred hospice or palliative care experience.</li><li>
Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.</li><li>
Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.</li><li>
Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.</li><li>
Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.</li><li>
Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.</li><li>
Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.</li><li>
Valid driver&rsquo;s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.</li></ul>
