Responsibilities
<ul>
<li>Under the direction of the RN, assist in identifying the physical and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations</li>
<li>Participate in planning and implementing care in conjunction with the RN</li>
<li>Provide care utilizing infection control measures that protect both the staff and patient</li>
<li>Assure continuity of quality patient care delivered with appropriate documentation</li>
<li>Prepare regular clinical/progress notes and summaries</li>
<li>Assist the patient with the activities of daily living while teaching appropriate self-care techniques</li>
<li>Provide and maintain a safe environment for the patient, including an optimum physical and emotional environment</li>
<li>Maintain client confidentiality</li>
<li>Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury, or significant change in the patient's condition</li>
</ul>
Qualifications
<ul>
<li>Graduate of an accredited school of vocational nursing</li>
<li>Proof of current LVN license</li>
<li>Proof of current CPR certification</li>
<li>Ability to work flexible schedule</li>
<li>Minimum 1 year experience as an LVN in a clinical setting preferred</li>
<li>Excellent interpersonal skills</li>
</ul>
Valid driver&rsquo;s license and reliable transportation</div>
