In this role, you'll work closely with individuals, families, guardians, and service providers to coordinate services, monitor outcomes, and ensure each person receives the support they need to thrive.</p><h3 class="jobSectionHeader"><b>
What You'll Do:</b></h3><ul><li>
Coordinate and monitor services for individuals on your caseload</li><li>
Advocate for individuals and help them access resources and supports</li><li>
Build strong relationships with families, guardians, providers, and community partners</li><li>
Assist with person-centered planning and goal achievement</li><li>
Serve as the primary point of contact and referral source for assigned individuals</li><li>
Ensure services are meeting needs and supporting independence, community integration, and quality of life</li></ul><h3 class="jobSectionHeader"><b>
What We're Looking For:</b></h3><p><b>
Required:</b></p><ul><li>
Bachelor's degree in Human Services or a related field</li><li><b>
OR</b> five (5) years of related, verifiable full-time experience in developmental disability services in lieu of a degree</li><li>
Minimum six (6) months of experience in developmental disability services</li><li>
Valid driver's license with a good driving record</li><li>
Ability to successfully complete the Targeted Case Management state test within the required timeframe</li></ul><p><b>
Preferred:</b></p><ul><li>
One (1) year of case management experience</li></ul><h3 class="jobSectionHeader"><b>
