</ul><div><b>ABOUT THE THE </b><b>ASSERTIVE COMMUNITY TREATMENT (ACT) TEAM</b></div>
<div><ul><li>The team serves the highest level of acute clients treated within the outpatient department at CMHC. ACT is a fidelity-based model that requires the Clinical Social Worker (CSW) to be part of the multidisciplinary team.</li>
<li>The CSW carries a caseload of clients assigned to them, assess them clinically, completes all required clinical paperwork, completes training needed to complete Emergency Certificates in the community ensuring that clients can be evaluated in the Emergency Department when needed for danger to self or others and grave disability.</li>

<div><p>In a state agency behavioral health program as part of a multi-disciplinary team at one or more locations this class is accountable for independently providing clinical social work services.</p>

OFFICE OF HEALTHCARE ADVOCATE: This class is accountable for serving as a case manager and advocate for clients that have received or need to receive behavioral health care services.</p>

<p></p><div><ul><li>As a member of multi-disciplinary clinical team or within a network of community-based behavioral health services develops psychosocial assessments through clinical interviews with assigned clients and families which include consideration of developmental, social and/or cultural, substance abuse, psychological, environmental and medical issues;</li>
<li>Develops treatment plans including measurable treatment objectives;</li>
<li>Provides clinical treatment services in the form of triage, crisis intervention, individual, group and/or family therapy, screening and planning;</li>
<li>Advocates and develops networks of social and clinical services to assist clients in meeting identified needs;</li>
<li>Monitors and evaluates effectiveness of treatment plans and documents treatment in clinical records;</li>
<li>Completes reports in accordance with agency and accrediting bodies;</li>

<li>Reviews and assesses medical records including psychological assessment and treatment plans, concerning but not limited to mental, emotional, behavioral and addictive disorders to determine appropriate treatment plans and eligibility for insurance benefit payment;</li>
<li>Participates in insurance appeal hearings and serves as clinical expert to defend and demonstrate that the diagnostic assessment and treatment plans are appropriate and required to address client behavioral healthcare needs;</li>

<div><ul><li><div>A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience.</div>
</li><li><div>A Master's degree in nursing may be substituted for one (1) additional year of the General Experience.</div>
</li></ul></div><h2 class="jobSectionHeader"><b>PREFERRED QUALIFICATIONS</b></h2>
<div><div>Preference will be given to candidates who possess the following experience:</div>
<div><ul><li>Experience working with individuals with Intellectual and/or Developmental Disabilities.</li>
<li>Experience with basic nursing skills to include but not limited to: providing medication and nutrition via G-tube or J-tube, diabetic care, respiratory treatments, colostomy, cecostomy, and urostomy care, wound care, and catheterization.</li>
<li>Experience following current infection control practices.</li>
<li>Experience with clinical documentation including documenting events in writing.</li>
<li>Experience working in community and family home settings and coordinating in home supports.</li>
<li>Experience reviewing health information and formulating reports based on that information.</li>
<li>Experience using Microsoft Office Suite including Outlook, Teams, Word, and Excel.</li>
<li>Experience communicating, interacting with, and explaining information to internal and external partners.</li>
