Responsibilities
<ul>
<li>Serve as the first point of contact for patients, offering professional, empathetic communication that fosters trust and comfort.</li>
<li>Provide administrative support to ensure the smooth operation of clinical services, including managing patient intake, scheduling, and data entry.</li>
<li>Address patient concerns and challenges with understanding and unconditional positive regard, supporting and empowering patients throughout their treatment journey.</li>
<li>Collect and accurately document patient details, medical history, and consent forms, ensuring accurate data entry into the Electronic Health Record (EHR) system.</li>
<li>Verify patient insurance coverage, assist with pre-authorizations, and ensure proper documentation for billing.</li>
<li>Ensure all patient records are complete, accurate, and up-to-date in the EHR system, including treatment plans and test results, while maintaining confidentiality.</li>
<li>Collaborate with clinical and administrative staff to ensure timely and efficient patient care, addressing needs and concerns as they arise.</li>
<li>Adhere strictly to HIPAA regulations, maintaining the confidentiality of all patient-related information.</li>
</ul>
Qualifications
<ul>
<li>High school diploma or equivalent.</li>
<li>Active Medical Assistant Certification</li>
<li>Administrative experience in a healthcare setting, ideally in behavioral health or SUD care.</li>
<li>Strong organizational skills and attention to detail, with experience in managing scheduling, data entry, and patient records.</li>
<li>Exceptional interpersonal and communication skills, with the ability to interact with patients in a compassionate, non-judgmental, and patient-centered manner.</li>
<li>Ability to manage sensitive or challenging situations with professionalism, patience, and empathy, ensuring all patients feel supported and valued.</li>
<li>Comfort with digital tools and systems, including medical office software and EHR platforms (e.g., Athenahealth), with the ability to quickly learn new technologies.</li>
<li>Experience with insurance verification and billing practices.</li>
<li>Passionate about Steadfast Health's mission to provide low-barrier, non-punitive, harm-reductionist SUD care.</li>
</ul>
This is a full-time, W2 position offering an hourly rate of $23-$28, commensurate with experience.

Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, with one hour of breaks each day.

About this Role</h3>

The Case Manager is a key member of the interdisciplinary treatment team, responsible for coordinating and advocating for comprehensive services that address the biopsychosocial needs of clients in outpatient substance use and co-occurring mental health treatment. Utilizing a trauma-informed, person-centered approach, the Case Manager supports client recovery through care coordination, resource navigation, and ongoing support to address barriers to treatment engagement and retention.

</ul><h3 class="jobSectionHeader">Key Responsibilities (include but are not limited to):</h3>
<ul><li>Conduct comprehensive case management assessments upon admission and at regular intervals.</li>
<li>Develop and implement individualized care plans in collaboration with the client and treatment team.</li>
<li>Coordinate access to social, financial, medical, legal, housing, and vocational resources to address social determinants of health.</li>
<li>Collaborates with Peer Support Specialists to work together in assisting patients in addressing their social determinants of health</li>
<li>Monitor progress toward care plan goals and update plans as needed.</li>
<li>Maintain ongoing communication with internal clinical team members and external service providers to ensure continuity of care.</li>
<li>Facilitate warm hand-offs and follow-ups to support treatment retention.</li>
<li>Provide psychoeducation and support on navigating systems such as Medicaid, legal aid, child welfare, etc.</li>
<li>Advocate for clients' needs while promoting autonomy and empowerment.</li>
<li>Maintain accurate, timely documentation in accordance with Medicaid, Medicare, and CARF standards (including contact logs, service plans, progress notes).</li>
<li>Participate in interdisciplinary team meetings, treatment planning, and staff development trainings.</li>
<li>Identify and address potential risks to client safety and report concerns according to protocol.</li>
</ul><h3 class="jobSectionHeader">Qualifications</h3>
<ul><li>Bachelor's degree in social work, Psychology, Human Services, or a related field preferred.</li>
<li>Case Management Certification or equivalent experience preferred.</li>
<li>At least one year of experience working with individuals with SUD and/or co-occurring disorders required.</li>
<li>Knowledge of trauma-informed care, harm reduction, and motivational interviewing principles.</li>
<li>Strong interpersonal, organizational, and advocacy skills.</li>
<li>Familiarity with state specific Medicaid documentation standards and local community resources.</li>
<li>Proficiency in EMR systems (Athena EMR experience preferred).</li>
</ul>
Schedule: This role follows the anticipated schedule of Monday - Friday, 8:30a - 5:00p EST. Please note that this schedule may evolve based on organizational needs, and flexibility may be required as Steadfast Health continues to grow.
