 Job Summary
 
 
 
 Burlington Health &amp; Rehabilitation Center is seeking a dedicated and compassionate Registered Nurse (RN) to join our team of healthcare professionals.  Responsibilities
 
 
 
 <ul>
 <li>
 Direct the daily activities of certified nursing assistants in accordance with current federal, state, and local regulations and guidelines, as well as established facility policies and procedures
 </li>
 <li>
 Ensure policies and procedures are complied with by nursing personnel assigned
 </li>
 <li>
 Participate in the admission, discharge, and transfers of residents as required
 </li>
 <li>
 Observe for changes in residents' status, notifying the physician and resident's family or representative, and documenting accordingly
 </li>
 <li>
 Transcribe physician orders to medical records and carry out orders as written
 </li>
 <li>
 Prepare and administer medications as per physicians' orders and observe for adverse effects
 </li>
 <li>
 Perform wound treatments as per physicians' orders, observe for changes, and document accordingly
 </li>
 <li>
 Schedules follow-up appointments for residents and transportation needs as indicated
 </li>
 <li>
 Provide nursing leadership to nursing personnel assigned to the unit
 </li>
 <li>
 Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures
 </li>
 <li>
 Perform rounds to ensure resident needs are being met and personnel are performing their assigned duties
 </li>
 <li>
 Collaborate with other members of the interdisciplinary team as needed to ensure residents' needs are holistically met
 </li>
 <li>
 Inform appropriate departments of concerns or complaints and file grievances as necessary
 </li>
 <li>
 Report any incidents or unusual occurrences to the supervisor, unit manager, assistant director, or nursing director and participate in the investigative process as needed
 </li>
 <li>
 Participate in end-of-life care as required
 </li>
 <li>
 Ensure there is adequate stock of medications, supplies, equipment, and notify appropriate personnel of needs
 </li>
 <li>
 Initiate, review, and update care plans as required
 </li>
 <li>
 Promote teamwork, mutual respect, and effective communication
 </li>
 <li>
 Participate in the survey process when inspection surveys arise
 
 
 
 </li>
 </ul>
 
 Requirements
 
 
 
 <ul>
 <li>
 Current unencumbered State of Vermont RN license
 </li>
 <li>
 Strong communication skills to communicate with residents, families, and staff
 </li>
 <li>
 Critical thinking skills
 </li>
 <li>
 Ability to handle different situations with tact

 Under the supervision of licensed nursing staff, the Certified Nursing Assistant (CNA) provides basic nursing care and assists residents with daily living activities in a compassionate, resident-centered environment in Walpole, MA.
 
 
 Responsibilities:
 
 
 <ul>
 <li>
 Answer call lights and respond to resident needs
 </li>
 <li>
 Assist residents with ADLs, including bathing, grooming, oral care, and incontinence care
 </li>
 <li>
 Assist with meals and feeding
 </li>
 <li>
 Assist with range-of-motion exercises, transfers, and wheelchair mobility
 </li>
 <li>
 Turn and reposition residents as needed
 </li>
 <li>
 Assist with short-distance ambulation within the facility
 </li>
 <li>
 Observe and report changes in resident's condition
 </li>
 <li>
 Perform other duties as assigned
 
 
 
 </li>
 </ul>
 Qualifications:
 
 
 <ul>
 <li>
 Valid CNA certification (required upon start date)
 </li>
 <li>
 High school diploma or equivalent preferred
 </li>
 <li>
 Six months of CNA experience in long-term care preferred
 </li>
 <li>
 Ability to communicate effectively with residents, families, and staff
 </li>
 <li>
 Ability to respond quickly and calmly in emergency situations
 
 
 
 </li>
