Why You'll Love This Role
 
You won't be stuck behind a desk all day. You'll be out in facilities, building relationships, solving problems, and making sure every resident's care journey is smooth and supported.
 
You'll play a critical role in: 
<ul>
<li>Coordinating all aspects of resident care and managing projects and programs offered to residents including but not limited to chronic care, and GUIDE</li>
<li>Guiding families through paperwork and insurance processes</li>
<li>Collaborating closely with clinical teams and intake coordinators</li>
<li>Arranging DME and ensuring non-clinical needs are handled efficiently</li>
<li>Being the steady, organized, compassionate presence families rely on</li>
</ul>

What We're Looking For
 
<ul>
<li>Someone who genuinely enjoys working with the elderly population</li>
<li>Highly organized and proactive — you stay three steps ahead</li>
<li>Comfortable making phone calls and communicating with families and facilities</li>
<li>Tech-comfortable (EMR experience a plus)</li>
<li>Willing to travel locally to facilities</li>
</ul>
Bonus Points For: 
<ul>
<li>Experience in case management or healthcare</li>
<li>BSW (preferred but not required)</li>
</ul>
