We offer pathways to housing for people experiencing homelessness through low-barrier access to wraparound services by providing innovative, trauma-informed, and person-centered programming that engages our communities and leads system-wide transformation. Through the Sunrise Hub day center, Sunrise Hotline call center, Sunrise Mobile outreach and shelter services, and Sunrise Housing program we serve nearly 30,000 people experiencing homelessness each year.Job Title: Shelter Case ManagerPosition Status: Full-TimeLocation: In-PersonTotal Compensation: $54,000-56,000 including unlimited PTO, Health/Retirement Reimbursement, Professional Development Stipend, Connectivity AllowanceFLSA Classification: ExemptPosition Summary: The Shelter Case Manager is responsible for providing compassionate and holistic support to individuals experiencing homelessness in a shelter setting. This role collaborates with our partner shelter organization to complete housing navigation tasks, primarily but not limited to managing a caseload of people experiencing homelessness who reside in the Southbridge shelter. The case management will include the completion of coordinated housing assessments, diversion/rapid exit, goal setting, resource coordination and assisting clients and shelter staff in navigating housing referrals to community partners.Position Qualifications: <ul><li>Bachelor&rsquo;s degree in Social Work, Psychology, Public Health, or a related field preferred.</li><li>Bilingual (preferred).</li><li>Minimum of two (2) years of experience working in a shelter environment, street outreach, housing navigation, or related setting (preferred).</li><li>Strong attention to detail with excellent time management, organizational, written, verbal, interpersonal, and computer skills.</li><li>Ability to work within a high-tolerance, harm reduction model serving individuals with multiple barriers to housing and stability.</li><li>Ability to learn quickly and work effectively with diverse populations, partners, and stakeholders.</li><li>Self-starter who can anticipate needs, stay ahead of deadlines, and thrive in a fast-paced environment.</li></ul>Position Responsibilities: <ul><li>Regularly meet with clients assigned to your caseload to build rapport, assess needs, complete goal planning, provide support, and track progress.</li><li>Maintain accurate and up-to-date documentation of client progress, services provided, case notes, and other relevant data in the Homeless Management Information System (HMIS).</li><li>Conduct comprehensive assessments of clients&rsquo; income, benefits eligibility, and available housing options to develop sustainable solutions.</li><li>Develop and implement individualized service plans aimed at securing and maintaining permanent housing for clients.</li><li>Assist clients with housing searches, ensuring options align with their specific needs and preferences.</li><li>Drive clients to and from services as needed to support completion of case management goals.</li><li>Work closely with case managers and community partners to coordinate services and ensure clients&rsquo; needs are met efficiently.</li><li>Participate in regular Sunrise staff meetings and training sessions to stay informed on best practices and organizational updates.</li><li>Assist in the training and onboarding of new case managers, including shadowing and mentoring as needed.</li><li>Perform other duties as assigned.</li></ul>Position Considerations: <ul><li>There are TWO different positions being offered. One position will be Monday through Friday; shifts occur between 8am-5pm and a second position that will include shifts happening Wednesday-Sunday, 11am-7pm.</li><li>Based out of the Southbridge shelter, located off I-35 in South Austin.</li><li>Rotating weekend shifts, holiday shifts, or irregular hours during extreme weather events may be required, but are not anticipated at this time.</li><li>Use of personal transportation to travel to and from locations is required; mileage will be reimbursed.</li><li>Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.</li><li>Work occurs in busy and loud environments.</li><li>May be exposed to environmental elements such as cold, heat, dust, noise, and odor.</li><li>May need to bend, stoop, twist, and sit throughout the day.</li><li>Works under the supervision of the Shelter Program Manager as part of the Shelter Services team and collaborates with TOOF staff.</li><li>May be required to attend various client staffing meetings in coordination with partner agencies.</li></ul>Sunrise Homeless Navigation Center values diversity and inclusion and is committed to creating a diverse and inclusive workplace.
