Identify on-going needs and services of residents in promotion of the highest quality resident services to be delivered.</li><li>

Assist Resident Care Director (RCD) in conducting resident assessments at time of move in, 30 days post move in, semi-annually and with any change in status.</li><li>
Conduct monthly wellness visits for all residents.</li><li>
Assess, monitor, and follow up with residents who have experienced any fall, accident, or incident and document and report as required and directed.</li><li>
Document all pertinent information in the resident wellness file.</li><li>
Maintain communications with resident&rsquo;s family and or responsible party to ensure medical needs for the resident are being met.</li><li>
Contact resident&rsquo;s attending physician when necessary and/or upon family request.</li><li>
Ensure weights and vital signs are obtained monthly for each resident.</li><li>
Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations.</li><li>
Maintain medical supplies and emergency kits for the community.</li><li>
Provide clinical support and assistance to community team members as needed.</li><li>
Understand and follow infection control practices.</li><li>
Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks.</li><li>
Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.</li></ul><h3 class="jobSectionHeader"><b>
Medication Programs</b></h3><ul><li>
Monitor each resident&rsquo;s medication and treatment profile regularly to ensure each medication and treatment is administered as ordered and documented accurately.</li><li>
Transcribe physician orders Medication Administration Record Treatment Administration Record (MAR/TAR) to include initials, date and time &lsquo;noted&rsquo; and, date faxed to pharmacy accurately and promptly.</li><li>
Complete MAR/TAR audit each month.</li><li>
Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass (&ldquo;Right&rdquo; resident, medication, dosage, time, route, right to refuse).</li><li>
Act as liaison for pharmacy services to ensure effective services for residents.</li><li>
Monitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.</li><li>
Complete Medication Observations and skills checks for medication care managers at the direction of the RCD.</li></ul><h3 class="jobSectionHeader"><b>

<h3 class="jobSectionHeader"><b>Recruitment Process</b></h3>
<ul><li>Partner with leadership team by championing the overall recruitment process.</li>
<li>Prepare and host job fairs, including advertisement, interview scheduling, and ensuring Department leaders are in attendance to interview qualified candidates.</li>
<li>Post positions and process candidates in the Applicant Tracking System (ATS).</li>
<li>Conduct candidate screening and interview qualified candidates.</li>
<li>Responsible for employment process per Sunrise Hiring Standards including background screenings, state/provincial/regulatory required screenings, setup drug/occupational health screens per Federal, State/Province, and Local regulations and Sunrise policy.</li>
<li>Conduct license verifications and other required verifications per Federal, State/Province, and Local regulations and Sunrise policy.</li>
</ul><h3 class="jobSectionHeader"><b>Team Member Onboarding and Team Member Management</b></h3>
<ul><li>Champion the team member on-boarding and welcome orientation process.</li>
<li>Ensure new team members are introduced to the community, set up with online training and facilitate system access and walkthroughs of key company applications (Workday, Amplify, OnShift, MyFlexpay, etc.).</li>
<li>Create and maintain team member personnel information in appropriate systems; Human Resources Information System (HRIS), ATS, Provider Trust.</li>
<li>Maintain physical and digital personnel files and records according to Federal and State/Provincial regulations and Sunrise policy.</li>
<li>Ensure that all team member changes (status, position information, pay, Leave of Absence, etc.) are entered into the appropriate systems accurately and in a timely manner.</li>
<li>Process and manage unemployment and worker&rsquo;s compensation claims and update claim activity as necessary.</li>
</ul><h3 class="jobSectionHeader"><b>Quality Assurance, Safety and Regulatory Requirements</b></h3>
<ul><li>Develop a thorough working knowledge of current and evolving state/province and federal regulations and ensure HR Compliance controls are followed in accordance with company policy and applicable regulations.</li>
<li>Maintain and organize audit materials and information for audit review.</li>
<li>Partner with community team to ensure compliance with national/provincial occupational health and safety requirements and promotion of Risk Management programs and policies.</li>
<li>Practice safety procedures at all times including use of Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.</li>
</ul><h3 class="jobSectionHeader"><b>Training, Leadership and Team Member Development</b></h3>
<ul><li>Lead all Human Resources system and policy trainings for front-line staff. An active participant and presenter at monthly Town Hall staff meetings with a focus on continued education targeting key business process workflows, policies, and systems.</li>
<li>Champion HR programs and new initiatives for all TMs.</li>
<li>Partner in the delivery and participation in Sunrise University Training and self-study programs during the required timeframe.</li>
<li>Maintain training compliance records, performance appraisal records, and ongoing data entry in Sunrise University portal.</li>
<li>Develop Team Member Engagement strategy and utilize YVC data to support the achievement of the Team Member Engagement goals and actively lead in the Engagement Planning sessions.</li>
<li>Coordinate the community performance management process and reporting. Process annual merit increases.</li>
<li>Be the HR partner to Department Coordinators when they hold their team accountable and correct actions when necessary and document for record keeping.</li><li>
Serve as a point of contact for employee relations matters, including conducting internal investigations, document findings, and support department coordinators in the application of corrective actions, ensuring compliance with Sunrise policies and applicable employment laws.</li>
<li>Attend regular meetings; Stand Up, Cross Over, Department Head, Town Hall, Quality Improvement, and others as directed by the Executive Director.</li>
<li>Keep abreast of professional developments in the field by reading and attending conferences.</li>
<li>Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.</li>
<li>Complete MKCC process monthly per policy.</li>
<li>Perform other duties as assigned.</li>

<li>Bachelor&rsquo;s degree preferred in business administration or related field</li>
<li>2+ years&rsquo; relevant experience in Human Resources, preferably in healthcare/Senior housing industry</li>
<li>Supervisory and management experience including hiring staff, coaching, performance management, discipline, and counseling</li>
<li>Proficiency with Microsoft Office (Windows, Word, Excel, PowerPoint, etc.)</li>
