We are currently seeking a skilled and experienced Nurse Practitioner to join our dynamic team to round in assisted living and independent living facilities.Job Description: As a Nurse Practitioner, you will play a crucial role in delivering high-quality primary care services to our patients. You will work closely with our physicians and healthcare team to diagnose, treat, and manage a wide range of medical conditions. Your primary responsibilities will include conducting patient assessments, developing treatment plans, prescribing medications, providing patient education and updating the families regarding any changes.Key Responsibilities:<ul><li>Conduct comprehensive patient assessments, including medical history and physical examinations</li><li>Develop and implement individualized treatment plans</li><li>Prescribe medications and other treatments as necessary</li><li>Order and interpret diagnostic tests</li><li>Provide patient education on health maintenance and disease prevention</li><li>Collaborate with physicians and other healthcare professionals to ensure continuity of care</li><li>Maintain accurate and detailed patient records</li><li>Participate in quality improvement initiatives and meetings</li></ul>Qualifications:<ul><li>Master's degree in Nursing/ Physician assistant from an accredited Nurse Practitioner/ PA program</li><li>Current Nurse Practitioner license in Illinois</li><li>At least two years of experience in a primary care/ geriatric setting</li><li>Strong clinical skills and knowledge of primary care practices</li><li>Proficiency in performing Pap smears, administering injections, and conducting IV infusions</li><li>Excellent communication and interpersonal skills</li><li>Ability to work independently and as part of a team</li><li>Commitment to providing compassionate and patient-centered care</li></ul>Benefits:<ul><li>Competitive salary</li><li>Comprehensive health insurance</li><li>Paid time off and holidays</li><li>Continuing education opportunities</li><li>Supportive and collaborative work environment</li></ul>Job Type: Full-timePay: $100,000.00 - $130,000.00 per yearBenefits:<ul><li>401(k)</li><li>Flexible schedule</li><li>Health insurance</li><li>Paid time off</li><li>Retirement plan</li></ul>Work Location: On the road
