Synchrony Rehab is seeking a licensed Physical Therapist or Physical Therapist Assistant to lead our rehab team as Therapy Program Director for The Village at Proprietor's Green, a dynamic and innovative senior living community in Marshfield, MA.</div><ul><li><b>
Setting:</b> Senior Living- outpatient</li><li><b>
Schedule:</b> Monday&ndash;Friday (no weekends or holidays)</li></ul><div>

Job Summary</b><br>
The Therapy Program Director (PD) is responsible for directing clinical assessments, program development, quality improvement, and productivity enhancement in a flexible interdisciplinary environment in campus location/s, home health, and/or outpatient care.<br>
Adheres to compliance with state licensure requirements and the company&rsquo;s policies and procedures, professional practices and ethical standards. Demonstrates accountability for and contributes to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary environment.</div><div></div><div><b><br>
Licenses and Certifications</b><br>
Must have and maintain a current, valid state licensure in respective discipline

Current valid CPR certification, preferred</div><div></div><div><b><br>
Qualifications</b><br>
<b>Education</b>: Degree in Physical/ Occupational Therapy, or Physical/ Occupational Therapy Assistant, or Speech Language Pathology from an accredited program

<b>Experience</b>: Minimum 1 year experience in delivering skilled rehab services</div><div></div><div><b><br>
Roles and Responsibilities</b><br>
<ul><li>Design and implement therapy programs and protocols that align with the facility&rsquo;s mission, goals, and standards of care.</li><li>Supervise and mentor a team of therapists, providing guidance, support, and performance evaluations to ensure high-quality patient care.</li><li>Monitor and evaluate the effectiveness of therapy programs, ensuring compliance with regulatory standards, accreditation requirements, and best practices.</li><li>Oversee the allocation and management of resources, including staffing, equipment, and budget, to support the successful delivery of therapy services.</li><li>Organize and facilitate ongoing training and professional development opportunities for therapy staff to enhance their skills and knowledge.</li><li>Analyze program data and outcomes to identify areas for improvement and implement strategies to enhance program effectiveness and patient satisfaction.</li><li>Meets the requirements of specific therapy discipline to deliver skilled rehab services within scope of credentials and/or license.

Physical Requirements</b><br>
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.</div><div></div><div><br>

Roles and Responsibilities</b><br>
<ul><li>Implement treatment plans designed by Physical Therapists, including therapeutic exercises, manual therapy techniques, and modalities (e.g., heat, cold, electrical stimulation).</li><li>Observe patient responses to therapy, document their progress, and report findings and any concerns to the supervising Physical Therapist.</li><li>Observe and maintain up-to-date documentation related to patients&rsquo; responses to therapy, document their progress, and report findings to the supervising Physical Therapist.</li><li>Provide instructions to patients and their families on exercise techniques, safety precautions, and strategies to support continued progress at home.</li><li>Support Physical Therapists in evaluating patient needs and progress, providing additional observations and insights.</li></ul>
