<div><div><div><div>We are seeking an intake worker for behavioral health and substance abuse intervention. The intake worker would be part of the behavioral health team, working in collaboration with providers and community health workers.</div>
<div></div><div><b>Intake Worker Responsibilities</b></div>
<ul><li>Conduct initial client assessments to gather detailed information</li>
<li>Evaluate clients' needs and determine eligibility for services</li>
<li>Identify appropriate levels of care and refer accordingly</li>
<li>Maintain accurate and up-to-date client records</li>
<li>Coordinate with other service providers and agencies</li>
<li>Follow up with clients to monitor progress and address any additional needs</li>
<li>Assist clients in completing necessary paperwork and applications</li>
<li>Ensure confidentiality and privacy of client information</li>
<li>Participate in team meetings and case conferences</li>
<li>Stay informed about community resources and services</li>
<li>Provide crisis intervention and support as needed</li>
<li>Advocate on behalf of clients to access necessary services</li>
<li>Develop and maintain positive relationships with clients and service providers</li>
</ul><div><b>Intake Worker Qualifications &amp; Skills</b></div>
<ul><li>Bachelor&rsquo;s degree in social work, psychology, or a related field</li>
<li>Experience working in a similar role or within social services</li>
<li>Strong knowledge of community resources and services</li>
<li>Excellent communication and interpersonal skills</li>
<li>Experience with case management software</li>
<li>Strong organizational and time-management skills</li>
<li>Ability to handle sensitive information with discretion</li>
<li>Proficient in Microsoft Office Suite</li>
<li>Excellent problem-solving abilities</li>
<li>Empathetic and non-judgmental attitude</li>
<li>Ability to work independently and as part of a team</li>
<li>Strong written and verbal communication skills</li>
<li>Experience in customer service or client support</li>

<b>POSITION SUMMARY</b><br>
We are seeking Behavioral Health Therapists for our clinic locations in Altgeld Gardens, Hegewisch, and Hammond. The Behavioral Health Therapist is a member of the interdisciplinary team and contributes to the comprehensive health care services offered by TCA Health. The Behavioral Health Therapist provides services to individuals, families, and groups to assist with improvement of social functioning and maximizing strengths. The Therapist interacts and interfaces with the community in diverse roles of advocacy and overall planning for service utilization. This position may be a regular full-time or a regular part-time employee. This position will be on-site.<br>

<b>Behavioral Health Therapist Responsibilities</b>
<ul><li>Perform bio-psychosocial assessments and diagnosis of behavioral health clients.</li>
<li>Provide effective treatment planning to assist clients in successfully achieving goals.</li>
<li>Evaluate crisis situations and apply appropriate interventions.</li>
<li>Assist in the detection of &ldquo;at risk&rdquo; patients and the development of plans to prevent further psychological or physical deterioration.</li>
<li>Initiate treatment interventions in individual, family, and/or group counseling settings.</li>
<li>Effectively and clearly document client interactions and referrals in a timely manner as required.</li>
<li>Completes all necessary statistical reports and attend to accuracy.</li>
<li>Regularly participate in Behavioral Health Department, TCA Health Staff, and other meetings as assigned.</li>
</ul>

<b>Qualifications</b>
<ul><li>Active Illinois License in good standing (LCSW or LCPC) for Altgeld Gardens and Hegewisch locations</li>
<li>Active Indiana License in good standing (LCSW, LCPC, or equivalent) if Hammond location is preferred</li>
<li>Master&rsquo;s degree in social work, counseling, or psychology</li>
</ul>
