<div>Direct Case Management Functions:
<ul><li>Establish rapport and working relationships with assigned clients, and work within a team approach to assist with all clients within the PSH program</li></ul>
<ul><li>Provide initial and ongoing assessment of client needs and strengths</li></ul>
<ul><li>Develop and write comprehensive client service plans with client input and conduct regular reassessments of needs</li></ul>
<ul><li>Assist with understanding Federal guidelines/expectations of rent subsidies, tenant obligations and landlord relationships</li></ul>
<ul><li>Assist with crisis prevention/intervention</li></ul>
<ul><li>Perform home visits to client&rsquo;s homes as needed and provide transportation for clients to secure benefits, find employment, obtain medical care and meet basic needs</li></ul> 


Maintains Client Contact and Client Centered Services:
<ul><li>Liaison with landlords to establish appropriate housing options for clients</li></ul>
<ul><li>Assist in linking client with community support systems and agencies</li></ul>
<ul><li>Advocate for clients&rsquo; rights and access to services</li></ul>
<ul><li>Assist in providing overall case coordination and management</li></ul>
<ul><li>Follow agency and legal standards regarding confidentiality</li></ul> 


Maintains Documentation and Other Methods of Communication:
<ul><li>Compliant with record keeping requirements that are consistent with program and agency standards</li></ul>
<ul><li>Maintains ongoing, accurate records and completes all documentation within set timeframes</li></ul>
<ul><li>Completes all required documentation including, but not limited to, assessments, service plans, progress notes, outcomes surveys</li></ul>
<ul><li>Attend and participates in staff meetings and Eastern Coordinated Access Network Meetings</li></ul>
<ul><li>Provide all information and timely updates directly to supervisor on all relevant issues</li></ul>
<ul><li>Database entry and use including HMIS and Ddap</li></ul>

Required Experience 
Bachelor&rsquo;s Degree in social work, human services or related field

OR 3+ years employment providing case management services

Possess computer literacy skills including Microsoft Office and Microsoft Word.

Possess current Driver&rsquo;s License and automobile.

Must be fully Covid-19 vaccinated or willing to complete series of vaccinations upon hiring.
