Position Type/Expected Hours of Work - Flexible to the needs of the program. Full time is 40 hours weekly. Part time is minimum of 20 hours weekly.
 
Qualifications - High school diploma or GED. Experience working with ID/DD population preferred. Valid/Acceptable New York State Driver’s License.
 
Relevant Certifications – Current New York State certification as a Certified Nurse Assistant.
 
Work Authorization/Security Clearance – Authorized to work in the United States.
 
Summary/Objective – This position assists in the coordination of the total day and residential programs for the People We Support stressing independence, inclusion and individualization with emphasis on the person’s choice. A person centered philosophy will be maintained. Must be progressive and proactive in providing for, arranging for, supervising and documenting services to People We Support to assist with promoting growth toward increasing levels of independence. Complete all required documentation in a timely fashion and in accordance with established agency guidelines and procedures. Will support the Agency’s values by working with measurable signs of PRIDE.

 1. Complete all required documentation in a timely fashion. Ensure that all services are documented and that all documentation is consistent with Waiver requirements, and with agency policy and procedure.
 2. Assist the People We Support in achieving their valued outcomes and life goals. Promote the development of functional skills.
 3. Provide guidance to People We Support with Doctors appointments and community activities; assist in arranging or providing transportation. Support staff shall be cleared by the transportation department to drive an agency vehicle and maintain their driving certification. If hired as non-driver or certification is lost, there will be a reduction in pay.
 4. Attend medical appointments, prepare appointment paperwork and be prepared to discuss medical issues and advocate for the People We Support.
 5. Assist with the maintenance of the Emergency Files. Will review monthly to ensure most current documents are included.
 6. Certified Nursing Assistant staff will assist with documenting fire drills as outlined by regulation and departmental procedure.
 7. Shall communicate all needs of People We Support to the appropriate Supervisor.
 
8. Shall participate in support staff meetings and other discussions regarding program needs and needs of People We Support.
 9. Promote community inclusion and relationships with non-disabled people as the focus of the programs.
 10. Provide adequate protective oversight at all times.
 11. Maintain a safe, comfortable and appropriate environment.
 12. Assist People We Support when crisis intervention may be necessary. Responsible for the implementation and follow through of all ADL, interpersonal skill training and/or support programs for people as outlined in each person’s service plans and Nursing Protocols.
 13. Shall attend, and demonstrate proficiency by written examination on all required in-services by achieving 80% competency on the exams.
 14. Responsible for maintaining current New York State CNA certification.
 15. Shall report any illness or accident, or changes, or problems in a person’s medical status to the nurse and shall use the appropriate documentation procedure and follow instructions as given.
 16. Communicate all matters concerning physical plant to the Supervisor.
 17. Certified Nursing Assistant will be required to accept temporary or permanent transfers within the department as necessary, in order to meet the needs of People We Support.
 18. If designated, shall be required to act as designated in charge in the absence of the Supervisor.
 19. Shall be aware of and follow correct procedures for the use of personal money for People We Support and program funds. Ensure proper purchasing of items and correct documentation on all ledgers.
 20. Participate with bus aide duty, as assigned. (Where applicable)
 21. Attend and satisfactorily complete all required in-services, including van certification.

Certified Nursing Assistant (CNA)

 Position Type/Expected Hours of Work &ndash; 7:30 am &ndash; 4:00 pm; 30-minute unpaid lunch: hours flexible to meet agency vehicle and transit transportation needs. Position includes on-call rotation.
 
 
 Qualifications - High School Diploma; Knowledge of DMV and DOT regulations pertaining to vehicle maintenance requirements. Minimum of two years&rsquo; experience with light/medium duty vehicle maintenance. Valid New York State Driver&rsquo;s License. Experience working with vehicle diagnostic equipment.
 
 
 Work Authorization/Security Clearance &ndash; Authorized to work in the United States.
 
 
 Summary/Objective - Responsible for mechanical repairs to all agency vehicles. Responsible to respond to any emergency calls. Responsible to comply with DMV Article 19-A and Commissioner&rsquo;s Regulations, agency policies, procedures and other items as assigned within the area of knowledge.
 
 
 
 Essential Functions &ndash;
 
 
 
 Responsible to complete, as scheduled, all requirements for compliance with Dept. of Transportation, Federal Transportation Administration, with a focus on 19-A regulations.
 
 
 Work in conjunction with other Mechanics, supervisors, and partner service centers to ensure maintenance on Agency and Transit vehicles are performed, as scheduled.
 
 
 Researching and preparing cost and time estimates using software for work scheduled and tracking scheduled, as well as completed maintenance work.
 
 
 In conjunction with other mechanics, will advise the Transportation Operations Manager of the general status and condition of all agency and transit vehicles, especially as it relates to safety issues and vehicle replacement due to repair.
 
 
 Maintain computer records on repairs, along with a schedule for preventative maintenance and regularly communicate via email. Keep database of parts and tools.
 
 
 Will comply with all aspects of the New York State Department of Motor Vehicle Inspection, DOT, and Repair Station regulations.
 
 
 Diagnosing vehicle issues manually or with the aid of software or digital tools to include inspections of vehicles for defects and repairs or replaces them.
 
 
 Complete diagnostics on engine control systems, anti-lock systems and Air Condition. Will test drive vehicles prior to and following repairs as needed.
 
 
 Completes required preparation work/repairs on Transit Buses prior to DOT inspections, as scheduled.
 
 
 Respond to fleet break downs, accidents, vehicle issues, and emergencies, as needed.
 
 
 Remain flexible to the needs of the Agency.
 
 
 Maintains the work area in a safe/clean manner, use tools and equipment in a conscientious and safe manner.
 
 
 Ensure only authorized personnel have access to the garage, garage equipment, tools, parts and supplies.

 Work Environment - Work is generally performed within an indoor garage and or outside when on callouts or emergency situations (parking lots, shuttle routes) in addition to some within an interior office environment, with frequent interruptions and irregularities in the work schedule. Work hours may vary and occasional evening and weekend hours may be required.
