Homedica is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide comprehensive psychiatric evaluation and medication management services across home-based, assisted living, skilled nursing, post-acute, and telehealth settings.
This is a full-time position offering a flexible schedule within a non-traditional care model, allowing you to practice with a high degree of autonomy while caring for medically complex and geriatric patients.
As part of an interdisciplinary team, you will serve as a behavioral health specialist, working alongside other providers to deliver integrated, patient-centered care.
Responsibilities:
<ul><li>Conduct comprehensive psychiatric evaluations and ongoing medication management
</li><li>Develop and adjust individualized treatment plans for conditions such as depression, anxiety, and dementia-related behaviors
</li><li>Collaborate with other providers and interdisciplinary care teams
</li><li>Provide consultative support to caregivers, facility staff, and community partners
</li><li>Deliver care across in-home, facility-based, and telehealth environments
</li><li>Participate in after-hours or on-call rotation as needed
 </li></ul>Qualifications:
<ul><li>Active and unrestricted Louisiana Nurse Practitioner license
</li><li>PMHNP certification required
</li><li>Active DEA license
</li><li>Strong clinical judgment and ability to work independently across diverse care settings
</li></ul>Preferred:
<ul><li>Experience managing behavioral health conditions in geriatric or medically complex populations
</li><li>Prior experience in facility-based, home-based, or post-acute care
</li><li>Familiarity with telehealth platforms and EMRs
