Position Summary</h2>
The Social Service Director oversees the planning, implementation, and evaluation of social service programs within a healthcare facility (such as a hospital, nursing home, or rehabilitation center). This role ensures that patients&rsquo; psychosocial, emotional, and practical needs are met in alignment with medical treatment goals and regulatory standards. The director also supervises social work staff and collaborates with interdisciplinary teams to support high-quality patient care.<h2 class="jobSectionHeader">
Key Responsibilities</h2><ul><li>
Program Leadership<ul><li>
Develop, implement, and evaluate social service policies and procedures.</li><li>
Ensure compliance with federal, state, and local healthcare regulations.</li><li>
Coordinate psychosocial assessments and discharge planning for patients.</li></ul></li><li>
Patient &amp; Family Support<ul><li>
Advocate for patients&rsquo; rights and dignity.</li><li>
Provide counseling, crisis intervention, and resource referrals to patients and families.</li><li>
Address issues such as adjustment to illness, grief, financial hardship, and community reintegration.</li></ul></li><li>
Collaboration &amp; Communication<ul><li>
Serve as a liaison between patients, families, medical teams, and community agencies.</li><li>
Participate in interdisciplinary care conferences and contribute to care planning.</li><li>
Build partnerships with external agencies for patient support services.</li></ul></li><li>
Team Management<ul><li>
Recruit, train, supervise, and evaluate social work staff.</li><li>
Provide mentorship, professional development, and performance feedback.</li><li>
Ensure adequate staffing coverage to meet patient needs.</li></ul></li><li>
Administrative Duties<ul><li>
Maintain accurate documentation in compliance with HIPAA and facility standards.</li><li>
Manage departmental budget and resource allocation.</li><li>
Prepare reports for senior leadership and regulatory agencies as required.</li></ul></li></ul><h2 class="jobSectionHeader">
Qualifications</h2><ul><li>
Experience: Minimum 3&ndash;5 years of social work experience in a healthcare setting; leadership/supervisory experience preferred.</li><li>
Skills:<ul><li>
Strong knowledge of healthcare regulations, discharge planning, and case management.</li><li>
Excellent interpersonal, organizational, and crisis management skills.</li><li>
Ability to lead teams, resolve conflicts, and foster collaborative working relationships.</li></ul></li></ul>
