Who We're Looking For</b></div><div>
The Case Manager ensures a safe and supportive environment for patients throughout treatment. This role assists with orientation, admissions, discharge planning, and documentation, while serving as a liaison between patients, the clinical team, and admissions staff.</div><div></div><div><b><br>
What You'll Do</b></div><ul><li>
Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.</li><li>
Coordinate and facilitate access to various services, including therapy, medical care, and community resources.</li><li>
Monitor clients' progress and adjust care plans as needed, ensuring effective treatment and support.</li><li>
Provide ongoing support and counseling to clients, helping them navigate challenges and maintain motivation.</li><li>
Collaborate with healthcare professionals, family members, and community organizations to ensure a cohesive support system.</li></ul><div></div><div><b><br>
What You Need</b></div><ul><li>
Knowledge of addiction and recovery preferred</li><li>
Knowledge of CARF, DCF and Joint Commission Standards, preferred</li><li>
Strong interpersonal and communication skills for working with patients, families, and interdisciplinary teams.</li><li>
Knowledge of mental health conditions, treatment modalities and community resources.</li><li>
Bachelor&rsquo;s degree in social work, psychology, counseling or related human services field preferred.</li><li>
Prior experience in behavioral health, mental health, or substance use treatment setting preferred.</li><li>
Current CPR Certification</li><li>
Crisis Prevention Intervention (CPI) Training</li><li>
