Summary</b></div><div>
This position is responsible for providing transition case management services to inmates in accordance with the requirements of the contract and the principles of the GEO&rsquo;s Continuum of Care. The key components of the GEO Continuum of Care include specialized staff training, enhanced GEOtrack case management, evidence-based interventions, education and vocational programs, transition planning, and post release support. This position is responsible for working with inmates within one year of release on the development of transition plans and connections to post release community-based resources. It will also provide individual cognitive behavioral treatment sessions and group sessions as warranted.</div><div><b>
Primary Duties and Responsibilities</b></div><ul><li>
Participates in appropriate training for designated responsibilities.</li><li>
Utilizes GEO&rsquo;s proprietary electronic case management system &ldquo;GEOtrack&rdquo; to record case management notes, track inmate progress with programmatic needs, transition services and final discharge planning with Continuum of Care team.</li><li>
Develops detailed knowledge of those assigned to specific caseload. Ensures academic, vocational, evidence based programming, support services and transition needs are addressed through case management plan.</li><li>
Develops transition plans in conjunction with inmates and makes appropriate linkages to community resources prior to release.</li><li>
Conducts individual and group cognitive behavioral treatment sessions as needed.</li><li>
Establishes a relationship with a statewide network of community-based service providers that address inmates&rsquo; needs and works with released inmates to eliminate barriers to successful reintegration.</li><li>
Actively engages community resources to work with released inmates emphasizing the critical first months of release. Works to connect community resources and pro-social people with inmates prior to release via telephone or through visits at facility.</li><li>
Works to ensure inmates obtain eligible benefits and proper identification prior to release.</li><li>
Assures timely communication and follow up with State personnel in the best interest of the inmate and successful reintegration.</li><li>
Invites community-based providers to participate in resource fairs at the facility.</li><li>
Gains an understanding of public domain services such as National Institute of Corrections, National Re-Entry Resource Center, and FDIC MoneySmart program.</li><li>
Reviews opportunities to transition inmates into their communities through community-based services such as transition centers and community residential centers.</li><li>
Attends scheduled staff meetings, promotes communication and the proper flow of relevant information between admission, staff and inmates.</li><li>
Compiles statistical data as needed for reporting purposes.</li><li>
Communicate effectively and coherently to administration, staff, inmates, visitors, and the general public, particularly in situations requiring tact, diplomacy, understanding, fairness, firmness and good judgment.</li><li>
Maintains appropriate documentation of inmate contacts as detailed in the agency program procedures.</li><li>

The Advanced Practice Provider diagnoses and treats acute, episodic, or chronic illness, independently or as part of a healthcare team. In addition, this position focuses on health promotion and disease prevention; order, perform, or interpret diagnostic tests such as laboratory work and x- rays; and prescribe medication.</div><div><b>

Performs routine medical examinations on inmates or detainees under established guidelines.</li><li>
Diagnoses and treats acute and chronic medical illnesses.</li><li>
Provides medical treatment and services, consistent with the provision of basic health care services, in conjunction with a physician.</li><li>
Formulates comprehensive treatment plans and prognosis based on the patient&rsquo;s health conditions.</li><li>
Prescribes medication dosages, routes, and frequencies based on patients' diagnosis permitted under State and DEA licensure.</li><li>
Orders, performs, or interprets the results of diagnostic tests such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays) with physician oversight as needed.</li><li>
Analyzes and interprets a patient&rsquo;s history, symptoms, physical findings, and diagnostic information to develop appropriate clinical assessment and treatment plan.</li><li>
Develops treatment plans are based on scientific rationale, standards of care, and clinical practice guidelines.</li><li>

Summary</b></div><div>
Under minimal supervision, this position investigates Workers' Compensation (WC) claims and Auto Liability claims and reports, and information provided to third-party administrators to ensure compliance with federal and state regulations and organizational policies. Monitors claim activities to conclusion in effort to reduce the financial impact to the organization.</div><div></div><div><b><br>
Primary Duties and Responsibilities:</b></div><ul><li>
Prepares, reviews, and submits reports regarding claims for Worker&rsquo;s Compensation and Auto Liability.</li><li>
Verifies data received from facilities, finance, and payroll departments for submission to insurance carriers on workers&rsquo; compensation.</li><li>
Coordinates and monitors the activity of vendor partners as it relates to individual claim outcomes.</li><li>
Monitors, maintains, and reviews all claims in designated states for timely and accurate reporting including, but not limited, to employee status, case reserving and litigated status.</li><li>
Maintains the Occupational Safety and Health Administration (OSHA) log module in Risk Management Information System (RMIS).</li><li>
Prepares, maintains and coordinates a variety of information with third party administrators including outstanding medical bills, employee lost time, employee return to work, and compensation data.</li><li>
Completes and maintains all forms and documents related to Workers' Compensation and Auto Liability.</li><li>
Works closely with the Human Resources Department and Director, Risk Management to assist in early claims reporting and for designated claims that are in the litigation process.</li><li>
Assists and participates in claims file audits.</li><li>
Assists and monitors the Modified Duty Program and all associated costs.</li><li>
Performs other duties as assigned.</li></ul>
Qualifications:
<div><b>
Minimum Requirements:</b></div><ul><li>
High school diploma or equivalent. Bachelor&rsquo;s degree desirable.</li><li>
Two (2) years of experience in <b>Workers' Compensation</b> claims management.</li><li>
Proficient with claims database applications.</li><li>
Demonstrated knowledge of claims adjusting principles and practices with emphasis on multiple-state WC law.</li><li>
Excellent verbal and writing skills.</li><li>
Demonstrate strong organizational and time-management skills.</li><li>
Ability to work with computers and the necessary software applications used by the department.</li></ul><div>

Help us bring the highest quality of healthcare to every patient by:</b></div><ul><li>
Sterilizing and laying out dental tools and instruments before scheduled procedures.</li><li>
Assisting with chair-side procedures such as keeping the operating field clear, passing instruments, suctioning, mixing materials, etc.</li><li>

Qualifications:

High school diploma or equivalent certification required.</li><li>
Graduate from an accredited dental assistant program and current license or certification (where applicable) to practice in the state in which the facility is located is required. Maintains a current copy of license on file at the facility at all times.</li><li>
A minimum of one (1) year experience as a Dental Assistant preferred.</li></ul><div><b>
Physical Requirements:</b></div><ul><li>
Lift or carry up to 20 lbs.; walk, bend, stoop, drive automatic equipment vehicles (61-100% of the time).</li><li>
Lift, carry, push, or pull up to 40 lbs.; stand, and climb (31-60% of the time).</li></ul><div>
