 About The ECM Program
 
 
 
 Our Enhanced Care Management (ECM) program delivers a whole-person, interdisciplinary approach to care, addressing the clinical and non-clinical needs of clients with complex medical and social conditions. The program focuses on:

 Job Overview
 
 
 
 Reporting to the Program Manager, the Case Manager delivers high-quality, client-centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15&ndash;20 participants, serving as their central point of contact.
 
 
 
 Key Responsibilities
 
 
 
 <ul>

 Conduct initial and quarterly psychosocial assessments
 </li>
 <li>
 Collaborate with participants on individualized service plans and housing stability goals
 </li>
 <li>
 Build trusted, professional relationships with clients
 </li>
 <li>
 Provide outreach, housing navigation, and ongoing support
 </li>
 <li>
 Facilitate referrals to healthcare, mental health, substance use, and community services
 </li>
 <li>
 Support clients in obtaining employment, benefits, and education opportunities
 </li>
 <li>
 Accompany clients to key appointments as needed
 </li>
 <li>
 Use a harm reduction, strength-based, and client-focused approach
