The Institute for Family Health

<div>Provides clinical mental health services to all age patients/families in the Institute for Family Health. The scope of practice includes but is not limited to diagnostic assessment, counseling, psychotherapy, brief treatment, behavior modification, crisis intervention and mental health consultation. The social worker will report to the Regional Director and/or Program Director and work collaboratively with members of the health care team throughout the organization, while utilizing independent judgment in delivering care and services. This position is a learned professional, requiring achievement of specific advanced educational training related to the duties detailed.<div>
<div>
<ul><li>Provide individual, family, and group counseling, crisis intervention and concrete services.</li></ul>
<ul><li>Work in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patients health problems.</li><li>
Prepares comprehensive bio-psychosocial evaluations, mental status exams, patient histories, treatment plans and reports.</li><li>
Conduct short and long term therapy with appropriate treatment plans, goals, and objectives.</li><li>
May supervise social work student interns.</li><li>
Educates staff regarding psychosocial issues related to patient care.</li><li>
Actively identify child welfare issues and coordinate with the Child Protective coordinator regarding appropriate reporting procedures.</li><li>
Document all patient services in patients medical records, completes encounter forms for all patient visits.</li><li>
Assure social service compliance with legal, regulatory and professional standards, including Article 28 and JCAHO (where applicable).</li><li>
Participates in social service quality assurance program.</li><li>
Participates in Social Service Department meetings and all required staff meetings.</li><li>
Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.</li></ul>

QUALIFICATIONS:
<ul><li>
Licensure in New York State as an LMSW, LCSW, LMHC-D
</li><li>
Masters in Counseling Clinical Psychology from an accredited University.
</li><li>
MSW with New York State Certification</li><li>
NYS licensure as LMSW, LCSW, LMHC-D</li><li>
NYS license if Clinical Psychologist or Psychiatrist</li><li>
CSW if MSW</li><li>
Strong diagnostic and clinical skills.</li><li>
Willingness to supervise interns and attend required supervisor courses</li><li>
Demonstrated experience working with diverse populations ie: HIV, homeless, mentally ill, elderly, children, adolescents, etc.</li><li>
Strong diagnostic and clinical skills</li><li>
Healthcare experience preferred</li><li>
Bi-lingual, Fluent Spanish preferred</li></ul>

MENTAL HEALTH CLINICIAN- Cadman Plaza

<div>The Family Nurse Practitioner insures delivery of comprehensive health care in accordance with the philosophy, standard of practice, objectives and policies of The Institute for Family Health. The Nurse Practitioner is responsible for diagnosis and treatment of common and chronic illness focusing on health promotion and disease prevention activities including patient education and counseling in the Institute of Family Health.
A. Clinical/Technical/Service
<ul><li>Demonstrates the ability to perform clinical/ technical/ service/ administrative tasks:
</li><li>Health Assessment, Treatment, and Maintenance</li><li>
Demonstrates competent level of technical skills.</li><li>
Conducts health assessment interviews.</li><li>
Performs complete physical assessment.</li><li>
Orders and interprets diagnostic tests.</li><li>
Develops plan of care based on the diagnosis and individual client needs.</li><li>
Prescribes medications and treatments, as appropriate.</li><li>
Works collaboratively with the health care team and makes referrals to other disciplines, as indicated.</li><li>
Records, reports, and interpret the client&rsquo;s responses to plan of care.</li><li>
Teaches, counsels and assists individuals and families to assume responsibility for prevention of illness and the promotion, maintenance and restoration of health.</li><li>
Collaborates with other members of the team in the provision of regular health screening and other health promotion measures.</li><li>
Identifies available community resources helpful to clients, refers and assists clients in utilizing such resources.</li><li>
Makes home/hospital visits to clients and/ or takes calls, as appropriate.</li><li>
Demonstrates the knowledge and skills necessary to provide care, based on physical, psychological, educational, safety and related criteria, appropriate to the age of the patients served in assigned area.</li><li>
Provide med phone coverage, as directed by supervisor.</li><li>
Provides feedback to Nurse Manager, Practice Administrator and/or Medical Director regarding policies and practices affecting the performance of the practice and/or their individual performance.</li><li>

EDUCATION REQUIRED
<ul><li>
Bachelor&rsquo;s degree and master&rsquo;s degree in nursing</li><li>
Graduate of an accredited Nurse Practitioner program</li><li>
Bilingual, Spanish preferred.</li></ul>

EXPERIENCES AND/OR SKILLS REQUIRED
<ul><li>
Strong FNP clinical experience.</li><li>
Basic computer skills.</li><li>
Ability to solve problems</li><li>
Leadership ability.</li><li>
Effective organizational and interpersonal skills</li></ul>

LICENSES/CERTIFICATIONS REQUIRED
<ul><li>
Current NY State Registered Nurse License and Registration.</li><li>
Nurse Practitioner Certification and Registration.</li><li>
Current Practice Agreement.</li><li>
Certified or eligible for certification by recognized specialty organization.</li><li>
Current BCLS, Infection Control, Child Abuse</li></ul>
