The Patient Service Representative (PSR) serves as the main point of contact for patients both in person and on the phone. The PSR communicates with patients in a warm friendly manner to obtain information needed for scheduling and registering patients. The PSR ensures that all demographics and insurance information is collected and correctly entered into Electronic Health Record (EHR). This role will support Warren Township school-based providers remotely.<br>

</b></p><p><b>Essential Duties and Responsibilities</b> include the following. Other duties may be assigned.</p>
<ul>
<li>Demonstrates exceptional customer service and communicates effectively with patients and patients' families.</li>
<li>Schedules patient appointments.</li>
<li>Interacts with patients in a friendly respectful manner.</li>
<li>Determines information needed for patient registration and provides appropriate documentation.</li>
<li>Determines and verifies insurance status and/or patient's eligibility for financial assistance programs and collects appropriate documentation from patients and patients' family.</li>
<li>Collects patient co-payments and practices proper money handling as defined by JPCHC standards.</li>
<li>Answers telephone calls by third ring and text messages within one hour.</li>
<li>Transfer calls appropriately.</li>
<li>Assists with medical record requests.</li>
<li>Prepares and mails patient correspondence as directed.</li>
<li>Responds to messages left within one business day.</li>
<li>Responsible for neatness and appearance of the patient waiting area.</li>
<li>Participates in all departmental and organizational meetings.</li>
<li>All other duties as assigned.</li>

Education and/or Experience</b></p>

<li>Knowledge of medical office practices</li>
<li>Excellent written and oral communication</li>
<li>Two years of experience in an ambulatory healthcare setting

<p>

The Patient Access Specialist ensures timely access to provider services by serving as the first point of contact for patients and referring providers. In this role, you'll answer and triage patient calls, schedule appointments, and verify insurance and demographic details all while providing a professional, supportive, and compassionate experience.</p>
<p>
This position has the potential for remote work, with reliable internet access required.</p>
<p><b>
Job Responsibilities (include but are not limited to):</b></p>
<ul>
<li>Answer, triage, and process requests for new patient appointments with both primary and specialty providers.</li>
<li>Schedule appointments that align with patient preferences and insurance requirements.</li>
<li>Collect and verify patient demographics and insurance information.</li>
<li>Coordinate with provider offices to ensure smooth transfer of patient information.</li>
<li>Accurately document scheduled appointments, referrals, and demographic data in the EHR.</li>
<li>Handle a high volume of requests efficiently while maintaining an excellent customer experience.</li>
<li>Promote teamwork, cooperation, and continuous improvement across departments.</li>
<li>Maintain professionalism in all interactions with patients, peers, providers, and leadership.</li>
</ul>
<p>Required Skills and Qualifications</p>
<ul>
<li>High school diploma or GED equivalent required.</li>
<li>At least 1 year of experience in a healthcare setting using computerized scheduling software.</li>
<li>Excellent computer skills with strong organization and attention to detail.</li>
<li>Previous call center experience preferred.</li>
<li>Bilingual English/Spanish strongly preferred. If bilingual, employees must complete the Bridging the Gap Program within one year of employment.</li></ul><p></p><div><br>

The Medical Director provides senior clinical leadership and oversight across assigned health center sites, including the 16th Street location. This role ensures high-quality, patient-centered care; strong provider performance; and consistent adherence to HRSA, UDS, Joint Commission, and value‑based care standards. The Medical Director partners closely with operational, nursing, quality, and behavioral health leaders to translate organizational strategy into reliable frontline practice. This is a highly visible leadership role ideal for a physician committed to safety‑net medicine, integrated care, and developing high-performing clinical teams.</div><br>

Clinical Leadership &amp; Oversight</b></div>
<ul>
<li>Serve as the senior clinical leader for assigned sites, supporting physicians, advanced practice providers, and multidisciplinary teams</li>
<li>Promote evidence-based medicine, standardized workflows, and patient-centered care</li>
<li>Support integration across primary care, behavioral health, dental, optometry, and enabling services</li>
</ul>
<div><b>Quality &amp; Performance Management</b></div>
<ul>
<li>Partner with Quality leadership to achieve UDS, HRSA, and payer quality benchmarks</li>
<li>Review and act on site-level dashboards and provider performance metrics</li>
<li>Lead quality improvement initiatives and corrective action plans</li>
<li>Support FTCA readiness through peer review, documentation standards, and risk mitigation</li>
<li>Lead Pediatric and Adult Clinical Councils related to P&amp;T, clinical policies, and procedures</li>
</ul>
<div><b>Provider Performance &amp; Development</b></div>
<ul>
<li>Conduct or support peer review, clinical coaching, and performance feedback</li>
<li>Participate in provider onboarding, mentoring, and remediation</li>
<li>Collaborate with the Chief Medical Officer on privileging, re‑credentialing, and professional standards</li>
</ul>
<div><b>Access, Throughput &amp; Clinical Operations</b></div>
<ul>
<li>Partner with Operations and Nursing to optimize access, panel management, and clinic flow</li>
<li>Address variation in rooming processes, visit types, same-day access, and care team workflows</li>
<li>Support implementation of new clinical programs and service lines</li>
</ul>
<div><b>Patient Safety &amp; Risk Management</b></div>
<ul>
<li>Participate in reviews of adverse events, near misses, and patient complaints</li>
<li>Lead root cause analyses and ensure timely follow-up</li>
<li>Implement corrective actions and coaching to improve safety</li>
</ul>
<div><b>Medical Staff Communication</b></div>
<ul>
<li>Serve as the primary clinical liaison between frontline providers and system leadership</li>
<li>Communicate organizational priorities, policy changes, and quality expectations</li>
<li>Elevate site-level issues, risks, and resource needs to the Chief Medical Officer</li>
</ul>

<div><b>Qualifications</b></div>
<div><b>
Education &amp; Experience</b></div>
<ul>
<li>MD or DO, board-certified in a primary care specialty</li>
<li>Minimum 5 years of clinical practice; leadership experience strongly preferred</li>
<li>Experience in FQHC, safety‑net, or Medicaid-focused environments</li>
<li>Demonstrated ability to lead clinicians, manage performance, and drive quality improvement</li>
<li>Experience with PCMH models preferred</li>
<li>Strong understanding of population health, disease management, and value-based payment programs</li>
<li>Proven success with pay‑for‑performance or quality incentive initiatives</li>
<li>Highly organized, detail-oriented, mission-driven, and an effective communicator</li>
<li>Proficiency with EHR systems and Microsoft Office</li>
</ul>
