Job Description Summary: 
The Housing Supportive Services Program (HSSP) is a specialized program that provides comprehensive and intensive field-based community mental health services designed to meet the unique needs of individuals in Permanent Supportive Housing (PSH) who are formerly homeless and have a Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED). As part of a multi-disciplinary team, the Clinician will conduct psychotherapy with adult members that have severe and persistent mental illness in the office, in member’s homes, or the field. The Clinician’s primary responsibilities are to engage members and to coordinate mental health and recovery services that will ensure housing stability and community integration. This position is responsible for maintaining the highest quality provision of direct individual and group therapeutic services and case management services, while fostering teamwork among staff and across departments and programs.
 
Job Description Essential Duties and Responsibilities: 
<ul>
<li>Provide, coordinate, and implement mental health and recovery support services with individual members.</li>
<li>Conduct individual, family and group psychotherapy.</li>
<li>Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health.</li>
<li>Collaborate with each member to develop their individual service plan; review and update it upon completion of goals.</li>
<li>Assist management and other Clinicians with program evaluation and modification for future program improvements and growth.</li>
<li>Assist members with becoming involved in daily activities, socialization, health maintenance, sense of community, and progress in recovery reflective of The People Concern’s model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation.</li>
<li>Work cooperatively and cohesively with other members of the HSSP team, including participation in weekly staff meetings, group clinical supervision, and trainings.</li>
<li>Develop and maintain a complete, accurate, and current member file with all funder-required documents.</li>
<li>Assist in crisis interventions as needed, including participating in on call rotation calendar for overnight and weekend After Hours Emergency Line.</li>
</ul>
Job Description Qualifications:

<li>Master’s degree in Social Work, Psychology, or a related field of study from an accredited university.</li>
<li>Valid registration or licensure with the California Board of Behavioral Sciences.</li>
<li>Current, valid California Driver’s with an acceptable driving record and reliable vehicle</li>
<li>Able to transport members in your personal vehicle</li>
<li>Outstanding clinical and strong team-building skills</li>
<li>Detail-oriented with excellent time management, organizational, written, verbal, and computer skills</li>

Job Description Summary: 
In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team the ODR Case Manager will work with clients recently released from jail, who are moving from homelessness to in permanent supportive housing, to provide intensive case management. Clients represent a diverse group with experiences of complex trauma, and who are facing medical, mental health and substance use barriers. The Case Manager collaborates with clients to learn independent living skills and other life-skills vital to success in maintaining permanent housing. Clients remain in the program for as long as they require subsidized housing.
 
Job Description Essential Duties and Responsibilities: 
<ul>
<li>Work with a diverse and marginalized client caseload of 15 justice-involved clients experiencing many barriers including history of homelessness, complex trauma, medical, mental health and substance use.</li>
<li>Maintain weekly contact with all clients and document contact in County approved database.</li>
<li>Ensure that case management services to clients are reflective of the “Whatever It Takes” model, which emphasizes customer choice, harm reduction, trauma-informed care and psycho-social rehabilitation.</li>
<li>Link clients to appropriate medical, mental health, substance abuse, and vocational resources.</li>
<li>Assist clients in obtaining all benefits to which they are entitled.</li>
<li>Participate in court hearings and submit court progress reports.</li>
<li>Ensure that each client on caseload has an up-to-date and comprehensive biopsychosocial assessment.</li>
<li>Collaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of goals.</li>
<li>Develop and maintain a complete, accurate, and current client file with all required documents and data in agency records and electronic databases.</li>
<li>Document all client contacts in PIP format (Purpose, Intervention, Plan) and in accordance with Housing Department productivity expectations.</li>
<li>Maintain confidentiality of client files per HIPAA and all applicable guidelines.</li>
<li>Responsible for coordinating appointments, transportation, and follow-up services for clients accessing primary health care, mental health care, recovery services, and other community resources.</li>
<li>Transport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegration.</li>
<li>Develop effective, trusting relationships with clients, with a focus on facilitating independence and maintenance of improved physical and mental health.</li>
<li>Maintain a current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to clients.</li>
<li>Assist clients with becoming involved in daily activities, scheduling and attending appointments, budgeting, socialization, health maintenance, sense of community, and progress in recovery.</li>
<li>Work cooperatively and cohesively with other clients of the staff team, including participation in weekly staff meetings and staff trainings.</li>

Job Description Qualifications:

<li>High School Diploma or GED or Equivalent</li>
<li>Bachelor’s degree preferred; or a minimum two years’ case management experience working with individuals experiencing homelessness, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience</li>
<li>Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record</li>
<li>Able to transport clients in your vehicle</li>
<li>Able to pass a background check for jail clearance</li>
<li>Demonstrated knowledge of case management</li>
<li>Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers</li>
<li>Skill in non-violent crisis intervention</li>
<li>Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs</li>
<li>Detail oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills</li>
<li>Able to obtain and maintain CPR/First Aid certification</li>

 Summary
 
 
 
 Case management utilizes a whole-person focus/approach when assessing client needs. This includes behavioral, physical, psychosocial, and activities of daily living. Case management also develops, coordinates, and assists with the implementation and facilitation of services for integrated care program consumers as defined by the individual plan of care; in alignment with being permanently housed.
 
 
 
 Essential Duties and Responsibilities:
