Job Description Summary: 
The Executive Assistant is responsible for providing professional administrative support to the Chief Executive Officer (CEO), Chief Administrative Officer (CAO), and Chief Operating Officer (COO), as well as the Board of Directors of The People Concern. The Executive Assistant ensures organized, efficient and professional operations and possesses the necessary skills to work effectively with senior leadership, staff, donors, and Board members.
 
Job Description Essential Duties and Responsibilities: 
<ul>
<li>Provide administrative support to the CEO, including but not limited to answering phones, detailed calendar management, expense report management, mail processing, invitation coordination, and travel coordination.</li>
<li>Coordinate meetings, including scheduling, attendee management, materials preparation, and room set-up for multiple Executive Leadership Team members and Senior staff.</li>
<li>Ensure Executive Conference Room remains straightened, clean, and stocked.</li>
<li>Edit Board of Directors meeting minutes accurately and in a timely manner, send Board meeting reminders and packets and respond to Board members’ requests for information.</li>
<li>Provide administrative support to the CAO and COO as needed, including but not limited to mail processing, phone screening, meeting scheduling, note taking and project management.</li>
<li>Assist with special projects and general office tasks as needed or assigned.</li>
</ul>
Job Description Qualifications: 
<ul>
<li>Bachelor’s degree or equivalent combination of education and experience</li>
<li>Minimum three (3) years’ experience working in Executive Administrative roles</li>
<li>Advanced Microsoft Office suite and Zoom skills.</li>
<li>Demonstrated client service orientation, with a positive attitude; exhibit a calm nature when encountered with customer service matters and client grievances; ability to appropriately represent Executive Management Team members with clear and professional communication</li>

Job Description Summary : 
As part of a multi-disciplinary team, the Clinical Case Manager will provide field-based and site-based clinical services with clients to help remove barriers to housing placement and retain permanent housing. The Clinical Case Manager’s primary responsibilities are to comprehensively assess initial and ongoing individual needs, build rapport and sustain therapeutic engagement, while implementing interventions guided by functional needs assessments and treatment goal plans. Clinical Case Managers utilize clinical knowledge and skills to create interventions that target growth in coping skills and other life-skills learning. Through the use of evidence based practices such as harm reduction, housing first, and trauma-informed care, Clinical Case Managers support housing stability, medical and mental health wellness, and community integration.
 
Job Description Essential Duties and Responsibilities: 
<ul>
<li>Maintain a caseload of approximately 20 high acuity Clients for intensive case management services (up to 40 if low acuity).</li>
<li>Develop relationships with Clients with a focus on facilitating independence and maintenance of improved physical and mental health.</li>
<li>Provide crisis assessment and intervention.</li>
<li>Facilitate life skills and coping skills groups for Clients, as indicated.</li>
<li>Contribute clinical insight during weekly team case conferencing.</li>
<li>Meet regularly with clinical supervisor and attend weekly group supervision in order to obtain hours towards licensure. Supervision is provided at no cost to employee.</li>
<li>Collaborate with LA County Department of Mental Health (DMH) and Department of Health Services (DHS) programs to enhance coordination within Clients’ treatment team.</li>
<li>Engage Clients and implement interventions with an approach emphasizing Client choice, harm-reduction, and psychosocial rehabilitation within a setting guided by trauma-informed care, housing-first principles, and professional boundaries.</li>
<li>Assist in the screening, intake, and enrollment of Clients including orientation to program policies and goals.</li>
<li>Complete comprehensive biopsychosocial assessments and functional needs assessments to guide individualized treatment planning.</li>
<li>Complete assessment of mental health symptoms and diagnoses to guide mental health service linkage.</li>
<li>Complete quarterly collaborative goal plans with Clients with the purpose of targeting areas that will improve quality of life and health outcomes.</li>
<li>Write and maintain accurate, complete, and up-to-date progress notes and data in electronic databases as required by the program and its funding sources.</li>
<li>Ensure all Client documents within physical charts are complete, accurate, current, and filed as guided by agency, department, and funder expectations.</li>
<li>Maintain confidentiality of Client information per HIPAA and internal agency guidelines.</li>
<li>Assist Clients in applying for and obtaining all benefits to which they are entitled.</li>
<li>Coordinate appointments, transportation, and follow-up services for Clients to improve access to primary health care, mental health care, substance use services, and other local resources in order to build sustainable community connections and reduce the need for emergency health care services.</li>
<li>Directly transport Clients in personal vehicle to various agencies and healthcare providers in the greater Los Angeles area to decrease barriers to access and increase community reintegration.</li>
<li>Support Clients by teaching and modeling life skills involved such as budgeting, housekeeping, meal preparation, appointment scheduling, health maintenance, and socialization within communities where they become housed.</li>
<li>Communicate effectively with property managers and other third party partners.</li>

 Summary
 
 
 
 Case management utilizes a whole-person focus/approach when assessing client needs. This includes behavioral, physical, psychosocial, and activities of daily living. Case management also develops, coordinates, and assists with the implementation and facilitation of services for integrated care program consumers as defined by the individual plan of care; in alignment with being permanently housed.
 
 
 
 Essential Duties and Responsibilities:
 
 
 
 <ul>
 <li>
 Serve a caseload of approximately 20-25 clients, providing active case management for all assigned cases, including meeting
 
 
 </li>
 </ul>
 weekly with each person individually
 
 
 <ul>
 <li>
 Act as the lead for client&rsquo;s housing-related case management, maintaining an active housing plan for each client on your
 
 
 </li>
 </ul>
 caseload, based in creative solutions to homelessness
 
 
 <ul>
 <li>
 Maintain current and thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around
 
 
 </li>
 </ul>
 services to , navigate them through the best options for their unique situations
 
 
 <ul>
 <li>
 Work collaboratively with clients to address barriers to housing, including linkages to income/benefits, mental health services,
 
 
 </li>
 </ul>
 physical health services, etc.
 
 
 <ul>
 <li>
 Develop effective, trusting relationships with clients and use a client-centered approach that includes motivational interviewing,
 
 
 </li>
 </ul>
 harm reduction, and trauma-informed care
 
 
 <ul>
 <li>
 Advocate for your client&rsquo;s needs in the shelter and when interfacing with other agencies or service providers
 </li>
 <li>
 Facilitate intakes for individuals who are referred into interim housing
 </li>
 <li>
 Create a supportive environment and ensure clients&rsquo; progress, utilizing a strengths-based approach
 </li>
 <li>
 Facilitate wellness, enrichment, and life skills groups
 </li>
 <li>
 Provide crisis intervention when needed
 </li>
 <li>
 Maintain client confidentiality at all times while following agency, state and HIPAA regulations
 </li>
 <li>
 Create and maintain accurate documentation of client information (intake, referrals, progress notes and service delivery) in HMIS
 
 
 </li>
 </ul>
 database and in accordance with program funder requirements

 Participate in case conferences, individual supervision, and other staff meetings
 </li>
 <li>
 Work in collaboration with other agency and program staff, in order to coordinate services
 </li>

 Communicate effectively and in a timely manner with management, peers, and clients
 </li>

 Participate in evaluation and trainings in order to better respond to client/community needs
 </li>

 </li>
 </ul>
 
 Qualifications:
 
 
 
 <ul>
 <li>
 Minimum of one (1) years of experience in case management required
 </li>
 <li>
 Valid CA Driver's license, reliable auto insurance and an acceptable driving record
 </li>
 <li>
 Experience and ability to demonstrate knowledge of issues faced by the population served (at-risk, experiencing homelessness,
 
 
 </li>
 </ul>
 and/or formally homeless)
 
 
 <ul>
 <li>
 Skilled in non-violent crisis intervention
 </li>
 <li>
 Ability to self-motivate and be flexible in a fast-paced environment with minimal supervision
 </li>
 <li>
 Strong interpersonal and team-building skills
 </li>
 <li>
 Strong knowledge of Microsoft Programs (Word, Excel, Outlook). *A computer skill test may be requested during the
 
 
 </li>

 Preferred Qualifications:

 Bachelor&rsquo;s degree in related field, strongly preferred but not required
 </li>

 Knowledge of HMIS databases and the Coordinated Entry System, is strongly preferred.
