Case Management Responsibilities (60%)
<br>Interviews applicants to determine eligibility for program enrollment based on established guidelines and
<br><b>requirements:</b> conduct needs assessment, obtains pertinent information and establish program goals for
<br>eligible clients; refers applicants to internal case management specialists, and/or other agencies if not
<br>appropriate for program.
<br>Develops client intake for clients; instructs clients in completing necessary paperwork including goals
<br>and/or budget forms; completes orientation check-off form and obtains release of information forms when
<br>appropriate.
<br>Plans, coordinates, and/or facilitates life management classes to assist clients in obtaining skills that will
<br>enable them to functionally cope with their environment; obtains speakers and workshop presenters as
<br>needed.
<br>Consistently serves as a mentor and provides positive examples of proper hygiene, personal appearance,
<br>records retention, budget planning, scheduling, etc.
<br>Refers clients to appropriate programs or other agencies based on clients needs and in accordance with
<br>their program plan.
<br>Completes discharge plans of clients exiting the program; transfers file contents to non-active file.
<br>Attends community meetings with representatives of other agencies to educate them about the program;
<br>makes client referrals and maintain awareness of client's progress.
<br>Interviews clients to determines appropriate, placement and/or referrals to other agencies.
<br>Co-facilitate Life Skill Groups; plans group activities and focus; provides group participants with
<br>information offered by other community services.
<br>Social Services Responsibilities (20%)
<br>Greets and interviews clients requesting assistance to determine client's eligibility for assistance based on
<br>program guidelines and restrictions, records clients disposition and other pertinent information.
<br>Assist Veteran’s Program Coordinator, Director of Operations and Director of Social Services in
<br>monitoring the Veteran’s Program Contract Requirements.
<br>Assist with conducting weekly shelter inspections
<br>Assist Veterans’ Program Coordinator with Weekly Medication Management.
<br>Transport Veterans to VA medical/benefits appointments at the VA in Salisbury.
<br>.
<br>Assist Veterans in locating landlords that will use their HUD VASH voucher (transporting once every 2
<br>weeks to look for housing).
<br>Assist Veterans with concerns as requested. Relay client issues and concerns to Coordinator of Veteran’s
<br>Program.
<br>Administrative Responsibilities (20%)
<br>Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and
<br>completeness of the same.
<br>Prepares and maintains client files documenting pertinent client information; prepares and maintains
<br>records of clients’ progress in files; completes assessment forms, monthly client statistics and progress
<br>notes; enters pertinent information into the established Homeless Management Information System
<br>(HMIS).
<br>Monitor supplies and make recommendations for reorders.
<br><br>
<b>Other Responsibilities:</b><br>
Assists with community service workers as needed.

<b>Knowledge, Skills, and Abilities:</b><br>
Knowledge of effective and efficient methods for organizing and maintaining records and ability to perform
<br>the same.
<br>Knowledge of the principles and practices of social service case management.
<br>Knowledge of community resources.
<br>Skill in Microsoft Word and Excel applications with the ability to effectively create and maintain
<br>Spreadsheets.
<br>Ability to interview clients to evaluate and serve their needs.
<br>Ability to work well with homeless veteran population.
<br>Ability to communicating in Spanish language is helpful.
<br>Ability to handle multiple duties simultaneously
<br>Ability to meet job attendance requirements as designated by day and shift
<br>Ability to pass drug screening test requirement
<br>Ability to interpret and implement programs to meet goals and objectives
<br>Ability to determine client eligibility for program assistance based on specific program guidelines and
<br>restrictions.

Case Worker (VA)

</div><p>The Office Coordinator will serve as the primary point of contact for visitors and callers to the Waukesha Corps while providing comprehensive administrative support. This role will facilitate effective communication within the office, support Corps Officers in their administrative needs, and ensure the smooth operation of the Corps Community Center.</p>

</p><p><b>Reception
</b></p><ul><li>Answer all telephone calls in a courteous and professional manner, transferring calls to appropriate personnel and taking messages as necessary.</li>
<li>Greet all guests warmly, providing information about the Corps Community Center's services and ensuring they are directed appropriately.</li>
<li>Maintain up-to-date knowledge of services offered at the Corps Community Center and provide referral information to other agencies as needed and directed.</li>
</ul>
<p><b>Administrative Support:</b></p>
<ul>
<li>Perform general office duties, including filing, handling mail, managing appointments, and general clerical assistance.</li>
<li>Support the Corps Officer(s) by performing their duties such as photocopying, assembling materials, filing, and preparing correspondence, collecting statistics.</li>
<li>Prepare and process mass mailings, thank-you letters, and other correspondence as needed.</li>
<li>Open the Corps Community Center building in a timely manner and assist clients and visitors in navigating the facility.</li>
</ul>
<p><b>Coordination of Community Services:</b></p>
<ul>
<li>Provides administrative support to the officers and leadership team to aid in coordination of Corps and Community Center programming, including the Red Kettle Campaign and Angel Tree.</li>
<li>Processes applications and ID for We Energies Positive ID.</li>
</ul>
<p>Education: High School Diploma or equivalent.</p>
<p>
Experience: At least 2 years in an administrative or receptionist role, preferably in a non-profit or social services setting.</p>
<p>
Certifications: Valid Wisconsin Driver's License with clearance to drive from TSA insurance is preferred.
