Summary :</h2>
The Home Health Case Manager (Remote) plays a crucial role in collaborating with health care team, patients/families and providers to assist with the plan of care and goal achievements. Responsible to coordinate and ensure care is safe, timely, effective, efficient, equitable, and patient-centered by performing at the top of the RN licensure. Furthers the professional practice of nursing at ThedaCare by promoting a culture of innovation and commitment to growth and professional development. The Remote Home Case Manager will be expected to train onsite and attend meetings, trainings and other events onsite at ThedaCare at Home as needed throughout the calendar year.<h2 class="jobSectionHeader">
Job Description:</h2>
Key Accountabilities
<ul><li>Coordinate patient care from a distance, primarily using phone or video communication, focusing on assessments, developing care plans, and ensuring patients access necessary resources and support</li><li>
Identify process improvement through data collection and auditing</li><li>
Skilled in navigating multiple EHR platforms and other applications</li><li>
Exhibits clinical excellence, strong communication and ensures the highest quality of care. Use home health expertise to triage patient needs and maintain open communication with patients, caregivers and other healthcare providers. Follow standardized triage protocols to ensure consistency in patient evaluation and delivery of care</li><li>
Manages holistic picture of individual/family/caregiver to promote overall health and wellbeing with focus on patient centered care</li><li>
Facilitates health optimization by progressing the plan of care from admission to discharge by determining the appropriate utilization of services</li><li>
Responsible for the collaboration between the interdisciplinary care team by identifying goals and coordinating the plan to ensure patient safety</li><li>
Creates engagement by taking responsibility for their professional development and other team members as evidenced by leading case conferences, assisting team member in-services, acting as a clinical resource and staff meeting input</li><li>
Demonstrate strong understanding of home health regulations, standards and OASIS RN clinic</li></ul>
Qualifications<ul><li>
Education &ndash; Bachelors of Science (BSN)</li><li>
Experience &ndash; minimum of 3 years home health bedside, EPIC, CIC, Remote Client, SHP, Rover experience preferred
</li><li>Licensure &ndash; Current Wisconsin RN license</li><li>
Certifications &ndash; American Heart Association Healthcare Provider Basic Life Support (BLS), OASIS Certification preferred</li></ul>

Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance</li><li>

KEY ACCOUNTABILITIES:<ul><li>
Performs delegated patient care activities using evidence-based practice and specific competencies for the patient population served.</li><li>
Checks patients&rsquo; vital signs, height, and weight, as needed, based on patient type.</li><li>
Provides compassionate care and listens to patients&rsquo; concerns and guides them by answering any questions or concerns.</li><li>
Identifies and escalates changes in patient status or patient risks to prevent crisis.</li><li>
Collects clinical data and information on patients and reports to care team.</li><li>
Repositions, transfers, ambulates and transports patients.</li><li>
Assists patients with basic needs (such as hygiene, eating, using the restroom, and dressing).</li><li>

Performs electrocardiograms (ECGs) as directed.</li><li>

Summary :</div>
The Accounts Receivable Specialist is responsible for submitting accurate billing to appropriate payers and actively following up on claims to ensure timely adjudication and reimbursement. This role communicates directly with commercial, governmental, and other third-party payers, as well as patients, guarantors, family members, and internal medical staff, to resolve claim issues and manage accounts receivable efficiently. The specialist identifies and resolves denials, underpayments, payment delays, and no-response claims while ensuring compliance with payer requirements and regulatory standards.<div>
Job Description:</div>
KEY ACCOUNTABLITIES:<ul><li>
Reviews, analyzes, and processes billed claims for accuracy upon submission, including charges, subscriber data, diagnosis and procedure codes, late charges, and supporting documentation.</li><li>
Submits claims in a timely manner in accordance with payer contracts, federal and state regulations, departmental standards, and form requirements.</li><li>
Follows up directly with commercial, governmental, and other payers via phone, correspondence, and electronic systems to resolve unpaid, underpaid, denied, or rejected claims.</li><li>
Identifies specific reasons for denials, underpayments, and payment delays; takes appropriate corrective action to secure reimbursement.</li><li>
Prepares, drafts, and submits technical and clinical appeals as needed, ensuring accurate and complete documentation.</li><li>
Re-bills accounts when new or corrected demographic, insurance, or third-party information is received and updates patient records accordingly.</li><li>
Verifies insurance, payer, and patient demographic information for accuracy at registration and during account follow-up, entering verification data into billing systems as required.</li><li>
Reviews internal and external reports to monitor claim status, payment variances, denial trends, and outstanding accounts.</li><li>
Maintains thorough documentation of all account activity, including payer contacts, phone numbers, correspondence, and actions taken, within the host system and/or tracking tools.</li><li>
Maintains a working knowledge of payer-specific requirements, contracts, and applicable federal and state regulations, applying them appropriately to account resolution.</li><li>
Identifies trends and root causes of accounts receivable issues and communicates findings and recommendations to management.</li><li>
Demonstrates initiative, effective problem-solving and analytical skills with the ability to determine appropriate collection strategies to resolve accounts.</li><li>
Ability to meet productivity and quality standards.</li></ul>
QUALIFICATIONS<ul><li>
One year previous experience in Revenue Cycle/Medical billing, follow up and collections or related degree/certification in Healthcare Management.</li><li>
Proficiency in basic computer applications, including Microsoft Excel.</li><li>
High School diploma or GED preferred.</li><li>
Strong verbal and written communication skills.</li><li>
Must be at least 18 years of age.</li></ul>
