Primary function is to administer skilled nursing care, under the supervision of a registered nurse for patients in their place of residence, coordinate care with the interdisciplinary team and patient/caregivers. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.</div></div><p>
</p><div><div><b>Qualifications:</b></div></div><ul><li><div>
Licensed Vocational Nurse, per the state requirements.</div></li><li><div>
1 year of Home Health experience</div></li><li><div>
Good communication skills, assessment skills, and strong clinical documentation skills.</div></li><li><div>
Ability to read, write, and speak the English language.</div></li><li><div>
Reliable transportation. Valid and current auto liability insurance.</div></li></ul><div><div><b>
Essential functions:</b></div></div><ul><li><div>
Under the direction of the RN, assist in identifying the patient&rsquo;s physical, psychosocial, and environmental needs.</div></li><li><div>
Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.</div></li><li><div>
Provide care utilizing infection control measures that protect both staff and patient (OSHA).</div></li><li><div>
Records treatments administered in medical record.</div></li><li><div>
Records clinical documentation according to accepted regulatory, corporate and professional guidelines.</div></li><li><div>
Participates in weekly case conferences.</div></li><li><div>
Participates in interdisciplinary communication to ensure that patients&rsquo; needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.</div></li><li><div>
Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.</div></li><li><div>
Participates in in-service training programs.</div></li><li><div>
Responsible for assuring patient safety.</div></li><li><div>
Complies with state regulations and national standards</div></li><li><div>
All documentation will be completed and synched by 9:00AM the following day, 7 days per week.</div></li></ul><p>

The DCS / DON coordinates care with the interdisciplinary team, patient/family and referring agency.</p><p>

Registered nurse (RN) licensed in Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)</b></div></li><li><div><b>
2 years of experience in a leadership role. (DCS, Clinical Supervisor, AdoN, etc)</b></div></li><li><div><b>
4 years of experience in Home Health
</b></div></li><li><div><b>HomeCare HomeBase experience.</b></div></li><li><div>
Good communication skills, assessment skills, and strong clinical documentation skills.</div></li><li><div>
Demonstrates proven decision-making skills.</div></li><li><div>
Nursing skills as defined as generally accepted standards of practice.</div></li><li><div>
Reliable transportation. Valid and current auto liability insurance.</div></li></ul><p>
</p><div><div><b>Essential Functions:</b></div></div><p>
</p><ul><li><div>Directs and coordinates clinical departments and branches; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations. (Conditions of Participation)</div></li><li><div>
Participates in activities relevant to professional services, including development of qualifications and assignment of agency personnel.</div></li><li><div>
Supervises and provides direction to subordinates, in an effort to ensure quality compliance with Plan of Care and assessment and reassessment of patients needs and continuity of services by appropriate health care personnel.</div></li><li><div>
Monitors QAPI Program and assures appropriate corrective measures are performed.</div></li><li><div>
Promotes compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary.</div></li><li><div>
Monitors systems and identifies problem areas to Administrator both verbally and through written reports.</div></li><li><div>
Meets mandatory continuing education requirements of the agency/licensing board.</div></li><li><div>
Promotes and educates regarding the concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.</div></li><li><div>
Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.</div></li><li><div>
