</p><div><div><b>Job Summary:</b></div></div><div><div>
The overall purpose of the <b>PT</b> position is to conduct all aspects of supervision and management of patient care, including PTA and HHA supervision, case conferences and discharge planning to enable the agency to identify the medically related social and emotional needs of the patients. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.</div></div><div><div><b>
Qualifications:</b></div></div><ul><li><div>
Physical Therapist, licensed per the state requirements.</div></li><li><div>
2 years of experience in Home Health preferred.
</div></li><li><div>Home Care Home Base experience preferred.
</div></li><li><div>Good communication skills, assessment skills, and strong clinical documentation skills.</div></li><li><div>
Ability to read, write, and speak the English language.</div></li><li><div>
Reliable transportation. Valid and current auto liability insurance.</div></li></ul><p>
</p><div><div><b>Case Manager Essential Functions:</b></div></div><ul><li><div>
Supervises and manages all aspects of patient care, including PTA and HHA supervision.</div></li><li><div>
Coordinates the distribution of visits among all disciplines (SN, PT, OT, ST, MSW, HHA) after the Start of Care, Recertification or Resumption of Care is completed, and coordinates POC with the physician. Ensures all visits are provided as ordered.</div></li><li><div>
Initiates referrals to appropriate ancillary services i.e. podiatry, community services.</div></li><li><div>
Facilitates interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.</div></li><li><div>
Leads weekly interdisciplinary case conferences for patient caseload, and contributes to the development of care plans for patients.</div></li><li><div>
Establishes specific social action plans and realistic goals for each patient consistent with medically related social and emotional needs.</div></li><li><div>
With the Administrator, interprets and promotes patients' rights and the &quot;Patients' Bill of Rights&quot;.</div></li><li><div>
Coordinates all aspects of discharge planning with patients, clinical team and agency.</div></li><li><div>
Provides individual and group help for patients and families at times of adjustment, crisis or particular need.</div></li><li><div>
Completes all necessary and supportive documentation-evaluations/re-evaluations, visit notes and discharges, in accordance with state and federal regulations.</div></li><li><div>
Ability to function as a team leader/role model.</div></li><li><div>
Serves on various committees as requested by Administrator.</div></li><li><div>
Has reviewed TheraCare Home Health Clinical Policies and Procedures on Abuse Prevention and knows the employee's responsibility to enforce it.</div></li><li><div>
Responsible for assuring patient safety.</div></li><li><div>
Performs other duties/tasks as assigned.</div></li></ul><p>
</p><div><div><b>PT essential functions:</b></div></div><ul><li><div>
Screens, evaluates, and treats patients.</div></li><li><div>
Evaluates patient within two working days of Start of Care.</div></li><li><div>
Completes a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.</div></li><li><div>
Reviews patient records for current diagnoses, past medical history, precautions and contraindications</div></li><li><div>
Establishes relevant, objective, measurable and functional goals with stated timeframes.</div></li><li><div>
Develops individualized plan of care and monitors progress toward stated goals</div></li><li><div>
Supervises and manages all aspects of physical therapy in patient care, including PTA supervision in accordance with state practice act. May also provide Home Health Aide supervision as needed per regulation.</div></li><li><div>
Documents all visits at point of care accurately and thoroughly, reflecting medical necessity and skilled need for physical therapy services according to accepted regulatory, corporate and professional guidelines.</div></li><li><div>
Maintains supervisory oversight for documentation completed by Physical Therapy Assistant(s).</div></li><li><div>
Performs re-evaluations as required to update and progress plan of care and provides updates to clinical team, physician, patient and caregiver as indicated.</div></li><li><div>
Participates in interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.</div></li><li><div>
Participates in weekly case conferences.</div></li><li><div>
Participates in in-service training programs.</div></li><li><div>
Establishes and instructs patients and caregivers in patient specific maintenance programs.</div></li><li><div>
Participates in all aspects of discharge planning with patients, clinical team and agency.</div></li><li><div>
Complies with state regulations and national standards as they apply to the practice of Physical Therapy.</div></li><li><div>
Responsible for assuring patient safety.</div></li><li><div>
Performs other duties/tasks as assigned.</div></li></ul><div><div><b><i>
