Position Overview</h3>
We are seeking a passionate and patient-centered Physical Therapist (PT) to join our growing team. In this role, you will deliver high-quality therapy services to residents in assisted living environments, helping them restore mobility, function, and confidence.<h3 class="jobSectionHeader">
Key Responsibilities</h3><ul><li>
Provide treatment visits, 30-day reassessments, re-certifications, and discharge evaluations</li><li>
Develop and implement individualized treatment plans with measurable short- and long-term goals</li><li>
Supervise and mentor Physical Therapist Assistants (PTAs)</li><li>
Manage a full caseload (28&ndash;32 visits per week for full-time positions)</li><li>
Maintain accurate documentation using an electronic medical record system</li><li>
Recommend and assist in fabrication of adaptive devices as needed</li><li>
Educate patients, families, and caregivers on therapy techniques and equipment use</li><li>
Collaborate with interdisciplinary healthcare teams to ensure coordinated, high-quality care</li></ul><h3 class="jobSectionHeader">
Qualifications</h3><ul><li>
Degree in Physical Therapy from an accredited program</li><li>
Active state Physical Therapist license</li><li>
CPR/BLS certification</li><li>
Valid driver&rsquo;s license and auto insurance</li><li>
Strong knowledge of medical terminology and therapy best practices</li><li>
Excellent communication and interpersonal skills</li><li>
Compassionate, empathetic approach to patient care</li></ul>
Preferred:<ul><li>
Geriatric experience</li><li>
Experience with care planning, discharge planning, or inpatient settings
