PRN Assessment RN (Contract) &mdash; Colorado NEW GRADS WELCOMEOverview Tree of Hope is hiring PRN Assessment Registered Nurses to perform home assessments across Colorado. This is a contracted position paying $125 per assessment. We offer flexible scheduling and will work around your availability. New grads are welcome to apply; mentorship available. Typical work split is ~80% field visits and ~20% office/home administrative time (varies seasonally). Position may be in-office or hybrid.Key Responsibilities<ul><li>Perform home health assessments (Start of Care, Recertifications, Discharges)</li><li>Complete change-of-condition visits</li><li>Conduct Pediatric Home Health Assessment Tool evaluations</li><li>Provide nursing interventions as indicated</li><li>Supervise and verify CNA skills</li><li>Obtain physician orders and coordinate care with the care team</li><li>Complete required documentation and administrative tasks remotely or in office</li></ul>Qualifications (Minimum)<ul><li>Active RN license (Colorado)</li><li>Minimum 1 year clinical experience in home health services or similar setting perferred</li><li>New grads welcome to apply; mentored/limited roles considered</li><li>Current BLS certification</li><li>Reliable transportation and ability to travel throughout assigned Colorado regions</li><li>Strong assessment, communication, and documentation skills</li></ul>Preferred<ul><li>Experience supervising CNAs</li><li>Pediatric home-health experience</li><li>Familiarity with home health regulations and documentation systems</li></ul>Compensation &amp; Work Arrangements<ul><li>$125 per assessment (contracted)</li><li>PRN schedule &mdash; flexible; we will work around your availability</li><li>In-office or hybrid options available</li></ul>How to Apply Send resume, current RN license, and brief availability to Intake@treeofhopecolorado.com.
