Tri-County Community Action Agency, Inc. (TCCAA) is a non-profit 501(c)(3) organization dedicated for over 50 years to making a positive difference in the lives of those experiencing hardships. The agency consists of multiple programs that are created to empower people in Halifax, Charlotte and Mecklenburg Counties of Southside Virginia. Through collaborative efforts, engaged staff and partnerships, Tri-County is able to further our mission to eliminate the violence of poverty through advocacy, empowerment and self-sufficiency.

What We Are Looking For:

The Housing Case Manager position requires a skill set that includes:
<ul>
<li>Working with homeless individuals/families and housing them as soon as possible and to provide a connection to community based supports that assist with sustainability.</li>
<li>Meet monthly goals; exercising good judgment; decision making that best utilizes Agency resources to benefit clients.</li>
<li>Coordination with a wide range of service organizations that promote self-sufficiency goals, community enhancement, and overall empowerment principles.</li>
</ul>
What You Will Do:

The primary duties and responsibilities consist of the following:
<ul>
<li>Intake process - Screening homeless clients seeking shelter using housing first assistance approach (assessments and intake applications).</li>
<li>Determining program eligibility (prevention, diversion, referrals, etc.).</li>
<li>Identifying the appropriate community resources that will assist with access and sustaining permanent rental housing as quickly as possible.</li>
<li>Providing information, diversion, referral services and/or shelter.</li>
<li>Program record-keeping (case notes, HMIS, CAP60, etc.).</li>
<li>Use the Voluntary Services model for all clients seeking services.</li>
</ul>
What You Will Need:
<ul>
<li>High level of professionalism.</li>
<li>Thorough knowledge of computer systems and use of the Internet in accessing client data and conducting client-related research.</li>
<li>Valid driver's record.</li>
<li>Knowledge of the housing first approach and emphasize stable, permanent housing as a primary strategy for ending homelessness.</li>
<li>Knowledge of outreach methods, interviewing skills, assessment techniques, and provision of emergency services and referrals.</li>
<li>Knowledge of a broad range of social service and or administrative aptitude.</li>
<li>Must be able to demonstrate knowledge and commitment to advocate for homeless individuals/ families as well as possess a thorough understanding and empathy for all persons in need.</li>
