 Experience

 12 months of role experience is required with some in the last 12 months.

 Must have at least 12 months of Case Management experience.
 </li>
 <li>
 Experience with Homecare Homebase (HCHB) is preferred from any number of months.

 </li>

 Local &amp; travel allowed.
 </li>

 Required Skills/Experience:
 
 1 Year of Hospice Nursing Experience Required.
 
 
 
 Shift &amp; Scheduling:
 
 8:30am-5pm (Schedule can be flexible based on patient&rsquo;s availability.) Weekends: One weekend per month. Holiday coverage required.
 
 
 
 Floating Requirements:
 
 Territory Details: Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy and neighboring cities as needed.
 
 
 7 days maximum time off
 
 
 1 reference from last 12 months (Manager/Supervisor reference type) - required for submission
 
 
 Driver&rsquo;s license required
