 Job Summary
 
 
 
 The Nurse reports to the Regional Nurse Supervisor and Program Director and will provide direct nursing support to the clients as well as education to the Treatment Team, to the clients, and to families of clients (as appropriate) in many areas, with special attention given to the role of psychotropic medications.
 
 
 
 Essential Job Responsibilities

 Provide high-quality nursing services to clients with Severe Mental Illness (SMI) and dual-diagnosis individuals including: medication education and support (scheduled, walk-in, individual, groups, crisis intervention, and on-call), consultation with Psychiatrist and physicians regarding clients and medication adjustments, facilitation of blood draw for labs as ordered, assistance with medical treatments/procedures as prescribed.
 </li>

 Per authority of license, chart appropriate records, review medicine orders regularly, obtain appropriate physician approval, and coordinate with pharmacy as needed.
 </li>
 <li>
 Observe client&rsquo;s physical condition and behavior, report significant changes to professional staff, and record clinical notes on clients.
 </li>
 <li>
 Responsible for monitoring and observation of clients self-administer medications; (as needed or requested) administration of prescribed injections as ordered by provider; and tracking of prescribed medication and dispensing as appropriate.
 </li>
 <li>
 Visiting clients in their homes, board and care facilities, or in other community placement locations to carry out prescribed treatment plans.
 </li>
 <li>
 Provide services to enhance clients&rsquo; development self-reliance in daily living and independent living skills.
 </li>
 <li>
 Order supplies and check delivery; stock and maintain inventory.
 </li>
 <li>
 Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of consumers.
 </li>
 <li>
 Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws.
 </li>

 Prepare and maintain a variety of records and prepare reports including documenting services to meet all clinical program and state requirements, including Short-Doyle/Medi-Cal requirements.

 Provide the Program Director with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.

 Complete the minimum required hours of training annually per Turning Point training requirements.

JOB SUMMARY

The Career Developer reports to the Program Director and is responsible for managing the career development services provided at the Male Community Reentry Program (MCRP). The MCRP is designed to support individuals transitioning from incarceration to community life through structured residential programming. The Career Developer plays a pivotal role in this process by delivering tailored employment and educational services that empower participants to achieve self-sufficiency and long-term success. This position is integral to helping residents identify and pursue vocational goals, secure employment, and engage in educational opportunities. Services are delivered through both individual and group sessions, customized to meet the diverse needs of the MCRP population. The Career Developer collaborates closely with substance use disorder (SUD) counselors, parole agents, and community partners to ensure participants are equipped with the tools, resources, and support necessary for a successful and sustainable reintegration into society.

ESSENTIAL JOB RESPONSIBILITIES

JOB SUMMARY

The Case Worker I, reporting to the Program Director, is responsible for providing comprehensive case management services. This includes supporting participants in their daily activities, assisting them in setting and achieving short-term and long-term goals, and guiding them toward stable, long-term housing and safe exits from the shelter. This role also involves transporting participants to and from essential appointments to ensure they have access to the resources needed for their success and well-being.
