JOB TITLE: Clinical Nurse CoordinatorEMPLOYER: Twelfth One, LLC dba Aspen InfusionDEPARTMENT: NursingREPORTS TO: Director of NursingLOCATION: Chandler, AZ (on-site)SUMMARY: A Clinical Nurse Coordinator provides administrative, clinical, and logistical support to infusion therapy teams, ensuring smooth and efficient patient care. Triaging patient clinical needs. Works collaboratively with other Aspen Infusion clinical and administrative departments. Responsible for conducting clinical referral review. Supports the Aspen Infusion Nursing team and partners with Agency nurses to ensure optimal patient outcomesDUTIES AND RESPONSIBILITIES:<ul><li>Serves as a subject matter expert and provides re-education and training to patients and caregivers on infusion therapy protocols.</li><li>Acts as a professional liaison between physicians, patients, and the care team.</li><li>Educates patients on protocol changes and safety measures.</li><li>Collaborates with physicians and assimilates information quickly.</li><li>Resolves clinical and administrative patient care problems.</li><li>Addresses technical and clinical infusion therapy issues.</li><li>Collaborate with interdisciplinary teams (clinical and nonclinical) to ensure optimal patient outcomes.</li><li>Provide patient education based on disease state, treatment, access, and adherence.</li><li>Assists patients with understanding infusion therapy procedures, answering questions, and coordinating access to necessary resources.</li><li>Supports sales staff in growing new business by accompanying them on sales calls to discuss clinical services and programs, contributes to the development of marketing tools and programs, marketing documentation, and clinical programs.</li><li>Answer incoming telephone calls as needed.</li><li>Provides effective and safe teaching using a patient-centered care approach for patients and families to achieve independence with prescribed therapy and care needs through active participation per the plan of treatment goals.</li><li>Respond promptly and appropriately to escalate requests and initiate emergency procedures as necessary.</li><li>Perform other related duties as assigned by management.</li></ul>SUPERVISORY RESPONSIBILITIES:<ul><li>This job has no supervisory responsibilities.</li></ul>RequirementsQUALIFICATIONS:<ul><li>Must possess a current Arizona RN license.</li><li>Must possess a current BLS certification.</li><li>Minimum of one year related experience or equivalent; infusion nursing experience is preferred.</li><li>Must possess a valid driver&rsquo;s license and auto insurance.</li><li>Proof of TB screening.</li><li>Strong charting/documentation skills.</li><li>Ability to understand and adhere to established policies, procedures, and protocols.</li><li>Commitment to excellence and high standards</li><li>Excellent written and oral communication skills</li><li>Strong organizational, problem-solving, and analytical skills</li><li>Ability to manage priorities and workflow</li><li>Versatility, flexibility, and a willingness to work within constantly changing priorities</li><li>Acute attention to detail</li><li>Ability to work independently and as a member of various teams and committees</li><li>Strong interpersonal skills.</li><li>Good judgment with the ability to make timely and sound decisions</li></ul>COMPETENCIES:<ul><li>Customer Service - Manages difficult or emotional customer situations; Responds promptly to customer needs; Solicits customer feedback to improve service; Responds to requests for service and assistance; Meets commitments.</li><li>Delegation - Delegates work assignments; Matches the responsibility to the person; Gives authority to work independently; Sets expectations and monitors delegated activities; Provides recognition for results.</li><li>Dependability - Follows instructions, responds to management direction; Takes responsibility for own actions; Keeps commitments; Commits to long hours of work when necessary to reach goals; Completes tasks on time or notifies appropriate person with an alternate plan.</li><li>Professionalism - Approaches others in a tactful manner; Reacts well under pressure; Treats others with respect and consideration regardless of their status or position; Accepts responsibility for own actions; Follows through on commitments.</li><li>Teamwork - Balances team and individual responsibilities; Exhibits objectivity and openness to others' views; Gives and welcomes feedback; Contributes to building a positive team spirit; Puts success of team above own interests; Able to build morale and group commitments to goals and objectives; Supports everyone's efforts to succeed.</li></ul>PHYSICAL DEMANDS AND WORK ENVIRONMENT:<ul><li>Continually required to stand, walk, sit, talk, and hear</li><li>Continually required to utilize hand and finger dexterity</li><li>Occasional exposure to bloodborne and airborne pathogens or infectious materials</li><li>While performing the duties of this job, the noise level in the work environment is usually moderate.</li><li>The employee must occasionally lift and /or move more than 30 pounds</li><li>Specific vision abilities required by this job include: Close vision; Distance vision; Depth perception and ability to adjust focus</li></ul>The above is intended to describe the general content of and requirements for the performance of this job.
