Our providers build long-term connections with their patients and lead efforts to reduce unnecessary hospitalizations through collaborative care.Position Summary: We are looking for a Physician (MD/DO) to lead rounding efforts in assisted living settings throughout the Twin Cities. You&rsquo;ll develop care plans, engage families, and collaborate with interdisciplinary teams&mdash;all in a flexible, mobile, and high-touch practice environment.Key Responsibilities:<ul><li>Visit patients on a regular schedule in designated senior living communities</li><li>Diagnose and manage both chronic and acute health conditions</li><li>Support end-of-life planning and advanced directives where appropriate</li><li>Collaborate with on-site teams to improve health outcomes and avoid unnecessary transfers</li><li>Foster meaningful patient-provider relationships</li></ul>Qualifications:<ul><li>MN-licensed MD or DO in good standing</li><li>Background in internal medicine, geriatrics, or family medicine</li><li>Passion for person-centered care and team collaboration</li><li>Reliable transportation and valid driver&rsquo;s license</li></ul>Join Our Team: Make a difference where it matters most&mdash;in the places seniors call home.
