Summary: 
 
Organizes and coordinates the activities and flow of work for the Program.

Participates on multidisciplinary teams to collaboratively address program development, problem identification and resolution, cost containment issues, regulatory compliance, and systems/performance/outcomes measures.
 
 
Facilitates the development, implementation, and evaluation of program services, practice standards, and initiatives, including program-specific committees, conferences, and staff/public education opportunities.
 
 
Serves as a resource to staff regarding clinical practice, policies, and program services. May provide direct patient care as needed to evaluate or assist with caring for patients. 
 
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action. 
 
Requirements: 
Minimum Required Education: Bachelor's degree in Nursing or ADN with BSN in progress.

Required Licensure/Certification: State licensure as a Registered Nurse (RN). Basic Life Support (BLS) Healthcare Provider and any relevant life support certification as determined at position level.

Coordinates the care of patients with chronic respiratory disease. 
 
Responsibilities: 
<ul><li>
Coordinates patient assessment and care, including collaborative care, case management, education, and discharge.
</li><li>Participates in multi-disciplinary meetings for the purpose of managing/reviewing patient care and treatment protocols, policies, and procedures.</li><li>
Serves as a resource to staff in areas of program development, clinical practice, and professional development.</li><li>
Assists with the development of educational materials for patients. Organizes and facilitates support groups for patients and their families.</li><li>

Requirements:

Graduate of an accredited Respiratory Therapy program.</li><li>
Registered as a Respiratory Therapist (RRT) by the National Board for Respiratory Care (NBRC).

BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire.</li><li>
