About the Medical Assistant role
 
Upperline Health is seeking an energetic, compassionate and driven Medical Assistant to assist our physicians in delivering complex health services within the clinic setting. This person primarily will work in our Greenwood, IN clinic Greenwood, IN clinic (Floating between Greenwood, Colombus, Seymour) and will be responsible for supporting the physician with a range of clinical and administrative tasks. Responsibilities will vary, and candidates must be flexible problem solvers and collaborative team players who are eager to learn and tackle problems. Ideal candidates will have podiatry or orthopedic experience and x-ray experience. 

Medical Assistant Responsibilities:

<li>Place patients in exam rooms; ensuring that they are ready and comfortable for their appointment with the physician</li>
<li>Complete exam intake, including but not limited to; reviewing medical history, social history, surgical history, and the history of present illness</li>
<li>Complete vitals and required documentation in clinic EMR (athenahealth)</li>
<li>Provide patient care under the direction of a podiatric physician including taking x-rays and assisting in the exam room as required</li>
<li>Perform daily administrative tasks such as re-stocking exam rooms and disposing of exam room trash</li>
<li>Responsible for cleaning and sterilizing exam and x-ray rooms both after each patient visit and at the end of each day</li>
<li>Equipment sterilization</li>
<li>Conduct health assessments and complete visits with Upperline Plus patients</li>
<li>Telephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative care</li>
<li>Care coordination activities including, but not limited to; medical records requests, order management, appointment scheduling, managing patient care referrals, etc</li>
<li>Travel to surrounding area clinics to provide support if needed</li>

Medical Assistant Qualifications:

About The Nurse Practitioner / Advanced Practice Provider (APP)
 
Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Orange Park, FL clinic. In collaboration with Upperline&rsquo;s local Physician team, APPs develop complex plans of care in accordance with the patient&rsquo;s health status and overall goals and values; provide medical diagnostics and treatments in the clinic, home, or telehealth, consistent with the APP&rsquo;s scope of practice and the patient&rsquo;s medical condition; educate patients and families on medication usage, diet &amp; nutrition, and crisis anticipation &amp; prevention. They will also communicate medical information to patients&rsquo; physicians, other providers and Upperline&rsquo;s Physician team and broader interdisciplinary team. Ideal candidates will have experience with the following: primary care / internal medicine, geriatric care and/or home-based care, palliative or hospice care and/or previously worked in a value-based care environment. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail, and thrive in a collaborative environment with rapid growth. Responsibilities:

<li>Develop and implement clinical plans of care for adult patients facing chronic conditions (e.g., diabetes, COPD, CHF, debility, dementia).</li>
<li>Gather patient history, physical exam, and diagnostics in the home or clinic setting.</li>
<li>Develop and implement treatment plans given patient&rsquo;s goals of care and current conditions.</li>
<li>Collaborate with the interdisciplinary team to meet the patient and family&rsquo;s physical as well as psychosocial needs.</li>

About the Compounding Pharmacy Technician Position 
Upperline Health&rsquo;s Ascend Pharmacy is looking for a full-time Compounding Pharmacy Technician to work in our non-sterile compounding lab. As part of a fast-growing company, this is an exciting opportunity to expand your pharmacy skills or gain hands-on experience with compounding medications.

We are a closed-door pharmacy with a Monday-Thursday 8am-4:30pm and Friday 8am-3pm schedule.

What You'll Do:

<li>Complete exam intake, including but not limited to, reviewing the patient's medical history, social history, surgical history, and the history of present illness
</li>
<li>Ensure accurate charting for the physician
</li>
<li>Provide patient care under the direction of a podiatric physician including taking and developing x-rays and assisting in the exam room as required
</li>
<li>Perform daily administrative tasks such as stocking all exam rooms and disposing of exam room trash
</li>
<li>Responsible for cleaning and sterilizing exam and x-ray rooms both after each patient visit and at the end of each day
</li>
<li>Sterilize instruments daily
</li>
<li>Satisfy and document appropriately required quality measures and audits
</li>
<li>Travel to surrounding area clinics to provide support if needed
</li><li>Support the connection of our patients to our extended care team of Nurse Practitioners and Care Navigators, where and when needed
</li>
</ul>Medical Assistant Qualifications:
<ul>
<li>Previous experience as a Medical Assistant; MA Certification is a plus
</li>
<li>Podiatry or Orthopedic clinic experience preferred
</li>
<li>X-ray experience is a plus
</li>
<li>Excellent computer skills and strong knowledge of medical terminology
</li>
<li>Previous experience working in an EMR &ndash; preferably athenahealth EMR
</li>
<li>Must be an effective communicator with excellent grammar and interpersonal skills
</li>
<li>Exceptional customer service orientation featuring an empathetic, compassionate, and professional demeanor with each interaction
</li>
<li>Demonstrated experience as a motivated and dedicated team member with a stable work history
</li>
<li>Reliable transportation and the ability to pass a background check and drug test is required
</li>
