We believe that everyone should have access to high-quality medical and dental care, regardless of ability to pay.Duties and responsibilities<ul><li>Provides primary medical care in accordance with the provider&rsquo;s medical specialty</li><li>Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary</li><li>Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner</li><li>Confers with other clinicians, patient&rsquo;s families and other support staff concerning treatment and care of patients when appropriate</li><li>Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care</li><li>Maintains accurate medical records within the EPIC electronic health record system and performs appropriate charting and follow up on patients</li><li>Participates in Medical &amp; General staff meetings and appropriate clinical teams</li><li>Participates in the Health Center&rsquo;s Performance Improvement program, in other task groups as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care</li></ul>Qualifications<ul><li>Graduation from an accredited medical school with a degree of Doctor of Medicine or Doctor of Osteopathy.</li><li>Completion of an approved internal medicine residency program</li><li>License to practice medicine in State of Connecticut</li><li>Ability and willingness to serve on the active medical staff of the hospitals used by the organization</li><li>Qualified in Basic Life Support techniques</li><li>Bi-lingual in Spanish and English is desirable</li><li>Experience in an ambulatory or outpatient setting preferred</li><li>Ability to relate with warmth and effectiveness to the patients and other staff of the Community Health Center.</li><li>Experience or specialty training in geriatric medicine is desirable</li><li>Doctorate</li><li>License to practice medicine in State of Connecticut</li><li>BLS/CPR</li></ul>Job Type: Full-timePay: $200,000.00 - $230,000.00 per yearBenefits:<ul><li>403(b)</li><li>403(b) matching</li><li>Dental insurance</li><li>Health insurance</li><li>Loan repayment program</li><li>Paid time off</li><li>Retirement plan</li><li>Vision insurance</li></ul>Work Location: In person

We believe that everyone should have access to high-quality medical and dental care, regardless of ability to pay.Job purposeTo provide high quality health care to the patients in the primary care setting, particularly, on high-needs elderly patients and other vulnerable adult populations.Duties and responsibilities<ul><li>Provides primary medical care by caring for adult and geriatric (&gt;60 years old) patients</li><li>Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary</li><li>Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner</li><li>Confers with other clinicians, patient&rsquo;s families and other support staff concerning treatment and care of patients when appropriate</li><li>Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care</li><li>Maintains accurate medical records and performs appropriate charting and follow up on patients</li><li>Participates in Medical &amp; General staff meetings and appropriate clinical teams</li><li>Participates in the Health Center&rsquo;s Quality Improvement program, in other task committees as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care</li></ul>Qualifications<ul><li>Graduation from an accredited medical school with a DO or MD degree.</li><li>Completion of an approved internal medicine residency program</li><li>Completion of a geriatrics fellowship or equivalent work experience managing elderly patients</li><li>License to practice medicine in State of Connecticut</li><li>Qualified in Basic Life Support CPR</li><li>Bi-lingual in Spanish and English is strongly desirable</li><li>Experience in an ambulatory or outpatient setting preferred</li><li>Ability to relate with warmth and effectiveness to the patients and other staff</li></ul>Job Type: Full-timePay: $210,000.00 - $245,000.00 per yearBenefits:<ul><li>403(b)</li><li>403(b) matching</li><li>Dental insurance</li><li>Health insurance</li><li>Loan repayment program</li><li>Paid time off</li><li>Retirement plan</li><li>Vision insurance</li></ul>Work Location: In person
