UR Medicine Home Care

<div><div><p><b>RESPONSIBILITIES:</b></p>
<div><ul><li>Assesses new cases for need and level of home care/nursing care required. Plans for comprehensive service to patients and their families: develops nursing care plans, including goals in conjunction with the patient&rsquo;s physician. Coordinates the patient&rsquo;s plan of care, including Home Health Aide and Personal Care Aide assignments. Evaluates the effectiveness of care plan, modifying the care plan as indicated. Communicates with the physician regarding the patient&rsquo;s condition and/or progress. Provides nursing care as follows:</li></ul>
<div><div><ul><li>Teaches and supervises the patient, family, PCA/HHA, and others providing nursing care or personal care to the patient.</li></ul>
<ul><li>Provides or assists with preventive and therapeutic treatments under a medical provider&rsquo;s direction. This may include patient transfers.</li></ul>
<ul><li>Initiates nursing measures to prevent complications to minimize disabilities.</li></ul>
<ul><li>Serves as the Case Manager/primary nurse to a caseload of patients.</li></ul>
<ul><li>Performs PRI/Screen and/or UAS as indicated.</li></ul>
</div></div><ul><li>Supervises care provided by Home Health Aides, Companions and PCA&rsquo;s and Private Duty Nurses as required. Assures that HHA services are consistent with agency vision, mission, values and strategic goals and meets customer&rsquo;s needs. Delivers HHA/PCA evaluations in a timely manner.</li></ul>
<ul><li>Responsible for approval of HHA/PCA time off requests and submission to payroll</li></ul>
<ul><li>Responsible for follow up on payroll questions by paraprofessional staff</li></ul>
<ul><li>Responsible for the disciplinary and corrective action plans as it relates to the paraprofessional clinical job performance.</li></ul>
<ul><li>Assists paraprofessional staff with documentation questions and fixes</li></ul>
<ul><li>Promotes good health practices to include understanding of diagnosis and implications involved. Promotes independence in self care. Provides information about and the coordination of available community resources.</li></ul>
<ul><li>Develops and revises comprehensive and coordinated plan of care in conjunction with patient, family, medical provider, care delivery team and third-party payers including obtaining medical orders and approval of medical services.</li></ul>
<ul><li>Maintains current knowledge of all available URMHC/Signature Care, MLTC and community-based services.</li></ul>
<ul><li>Supervises care and the care plan provided by Personal Care Aides/Home Health Aides.</li></ul>
<ul><li>Maintains timely and accurate reports and records, adhering to the agency standards.</li></ul>
<ul><li>Ensures the timely and appropriate resolution of all questions, complaints, and emergencies per agency standards</li></ul>
<ul><li>Participates in activities that promote agency and staff growth and development,</li></ul>
<ul><li>Participates in Quality initiatives, including clinical record audits and accrediting body surveys.</li></ul>
<ul><li>Participates in programs for the development of clinical expertise.</li></ul>
<ul><li>Participates on committees as assigned.</li></ul>
<ul><li>Assures that care delivery services are consistent with the ICARE goals, agency vision and customer needs and investigates patient complaints and incidents as assigned and develops improvement plan as indicated.</li></ul>
<ul><li>Responsible for the Key Performance Indicators (KPI&rsquo;s) established by leadership including timeliness of documentation and productivity standards.</li></ul>
<ul><li>Responsible for managing assigned Work Ques and Reports in eRecord related to paraprofessionals, as assigned</li></ul>
<ul><li>Participates in scheduling of paraprofessional staff in collaboration with PSC&rsquo;s and LHCSA Manager</li></ul>
<ul><li>Serves as back up the LHCSA Manager</li></ul>

<p></p><p><b>QUALIFICATIONS:</b></p>
<div><ul><li>Current NYS RN licensure in good standing. PRI/Screen and UAS certification preferred, but will train.</li></ul>
<ul><li>Minimum of one-year nursing practice in a hospital or home care setting.</li></ul>
<ul><li>Management Experience Preferred.</li></ul>
<ul><li>Knowledge of medical and surgical nursing practice.</li></ul>
<ul><li>Ability to apply technical nursing procedure safely.</li></ul>
<ul><li>Ability to communicate effectively, on a verbal and written basis.</li></ul>
<ul><li>Ability to utilize supervision and consultation constructively.</li></ul>
<ul><li>Ability to effectively supervise paraprofessional staff.</li></ul>
<ul><li>Ability to work with individuals from differing socio-economic backgrounds.</li></ul>
<ul><li>Ability to apply effective teaching techniques.</li></ul>
<ul><li>Must have a safe driving record, as defined in URMHC Policy, and a valid NYS driver&rsquo;s license.</li></ul>
<ul><li>Must have a telephone and reliable transportation.</li></ul>
<ul><li>Basic computer skills required.</li></ul></div><br>

Registered Nurse - Per Diem LHCSA

<div><p><b>Position Summary: </b>Provide triage for patient/family phone calls and coordinate any necessary follow up with the Interdisciplinary Group (IDG) team. Initiates home care referrals over the phone from physician offices, hospitals, SNF, community agencies and community at large. Serves as a customer resource and arranges appointments for evaluation as necessary. Provides comprehensive support for field clinicians, patients, families, providers, managers and internal staff.</p><br>

<div><p><b>Minimum Qualifications:</b></p>
</div>
<ul><li>A graduate of an approved school of professional nursing, who is a Licensed New York State Registered Nurse (RN)</li>
<li>Experience in interviewing, assessment, communication, negotiating and decision-making skills.</li>
<li>Minimum of one year of experience in Home Care in a CHHA or related experience. Hospice experience preferred.</li>

<div><p><b>Responsibilities
</b></p></div><p><i>Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodation:</i></p>
<p><b>Triage:</b></p>
<ul><li>Triages calls from hospice patients and families/caregivers to gather information related to patient/family needs and/or symptom management.</li>
<li>Provides nursing education to patients/caregivers based on patients&rsquo; plan of care and IDG recommendations.</li>
<li>Addresses medication inquiries/refills/changes, DME and supply needs, or need for home visit by a hospice nurse.</li>
</ul>
<p><b>Referral Management</b><b>:
</b></p><ul><li>Obtains and completes referrals over the phone for UR Medicine Hospice services. Identifies and documents the hospice focus of care, hospice diagnosis, prognosis, and essential demographic information.</li>
<li>Ensure insurance payors have been verified.</li>
<li>Communicates and coordinates referral process with the interdisciplinary team at UR Medicine Hospice and at institutions, as well as with patients/families. Involves patient and family support system in the referral process to assure the maximum involvement in patient care needs at home, if appropriate.</li>
<li>Ensures documentation meets regulatory, institution and agency/departmental standards.</li>
<li>Referrals are timely, accurate, and complete and communicated to the hospice team.</li>
<li>Electronic Medical Record (EMR) activities to include referral entry, name changes, demographic changes, NTUCs and processing deaths.</li>
</ul>
<p><b>Scheduling:</b></p>
<ul><li>Coordinates field staff schedules utilizing EMR scheduling tool. Ensure patients are scheduled for home visits according to departmental standards and processes.</li>
<li>Assist nurses with arranging unscheduled visits daily as needed.</li>
<li>Schedule and/or assist nurses with scheduling evaluation and admission visits for hospice enrollments in all settings as needed.</li>
<li>Provides weekend staff with scheduling needs to ensure timely admissions and/or patient visit needs.</li>
</ul>
<p><b>Compliance:</b></p>
<ul><li>Comply with agency and regulatory standards.</li>
<li>Completes required/mandatory in-services, professional licensing requirements, employer and institutional requirements.</li>
<li>Answers phone calls within agency standard guidelines.</li>
<li>Read and respond to emails throughout workday.</li>
<li>Comply with Corporate Compliance standards including confidentiality and HIPPA requirements.</li>
<li>Comply with safe work practices (ergonomics, OSHA).</li>
<li>Shares in responsibility for team maintenance of logs, statistics, and report corrections.</li>
<li>Take responsibility for professional and personal growth and development.</li>
<li>Attend team meetings and in-services.</li>
<li>Participates in mentoring/training new staff.</li>
