Innovative Urban Village, opening in Summer 2026, is URI&rsquo;s first supportive housing program, developed in partnership with Christian Cultural Center. Located in East New York, Brooklyn, Phase 1A includes 376 income based rental residences, with 94 units dedicated to families who have experienced homelessness due to domestic violence and who qualify for ESSHI services. The program provides permanent supportive housing and trauma informed, culturally responsive services designed to promote long term housing stability and independent living. As URI&rsquo;s inaugural supportive housing initiative, Innovative Urban Village requires the development of a comprehensive program model, policies, procedures, and service framework from inception through full implementation.

POSITION OVERVIEW

The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.

This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.

Salary: $60,000 - $70,000

MAJOR DUTIES AND RESPONSIBILITIES

Direct Service &amp; Case Management
<ul>
<li>Conduct comprehensive intakes, biopsychosocial assessments, and individualized service plans for assigned residents.</li>
<li>Provide supportive counseling, crisis intervention, and safety planning using trauma‑informed, survivor‑centered approaches.</li>
<li>Facilitate groups and workshops focused on parenting, wellness, DV recovery, tenancy skills, and life skills.</li>
<li>Conduct required home visits to assess safety, tenancy conditions, and service needs.</li>
<li>Provide advocacy and accompaniment for residents navigating benefits, legal systems, education, childcare, and healthcare.</li>
</ul>
Housing Stability &amp; Tenancy Support
<ul>
<li>Support residents in maintaining long‑term housing stability through proactive engagement and early identification of risk factors.</li>
<li>Educate residents on lease compliance, communication with property management, conflict resolution, and household management.</li>
<li>Collaborate with Housing Specialists and property management to address tenancy concerns and prevent eviction.</li>
<li>Assist residents with recertifications, inspections, and housing‑related documentation.</li>
</ul>
ESSHI, OTDA &amp; Supportive Housing Compliance
<ul>
<li>Ensure all services align with ESSHI service categories, including case management, employment support, benefits access, crisis intervention, and tenancy support.</li>
<li>Maintain accurate and timely documentation in CAPS, HMIS, and other required data systems.</li>
<li>Complete service plans, progress notes, assessments, and incident reports in accordance with agency and funder standards.</li>
<li>Participate in audits, case reviews, and quality assurance activities.</li>
</ul>
Crisis Intervention &amp; Risk Management
<ul>
<li>Respond to crises using de‑escalation, safety planning, and trauma‑informed strategies.</li>
<li>Conduct risk assessments related to DV lethality, mental health, child safety, and tenancy stability.</li>
<li>Coordinate emergency responses with internal leadership and external partners, including ACS, NYPD, mobile crisis teams, and hospitals.</li>
</ul>
Resource Navigation &amp; Community Linkages
<ul>
<li>Connect residents to community‑based services including mental health treatment, substance use programs, childcare, legal services, and medical care.</li>
<li>Provide benefits navigation for SNAP, Medicaid, SSI/SSD, childcare subsidies, and other entitlements.</li>
<li>Support residents in accessing education, vocational training, and employment programs in collaboration with the Economic Empowerment Specialist.</li>
</ul>
Interdisciplinary Collaboration
<ul>
<li>Participate in interdisciplinary case conferences and service coordination meetings.</li>
<li>Collaborate with the Director of Clinical Services to ensure clinical needs are addressed and referrals are completed.</li>
<li>Work closely with Housing Specialists to support move‑ins, housing retention, and tenancy success.</li>
<li>Maintain strong communication with operations and property management teams.</li>
</ul>
Training &amp; Professional Development
<ul>
<li>Participate in ongoing training on trauma‑informed care, motivational interviewing, DV best practices, crisis intervention, and supportive housing standards.</li>
<li>Engage in reflective supervision and contribute to a culture of learning and accountability.</li>
</ul>
SPECIFICATIONS FOR EDUCATION/CERTIFICATIONS/LICENSES
<ul>
<li>Bachelor&rsquo;s degree required.</li>
<li>Master&rsquo;s degree in Social Work or related field preferred.</li>
</ul>
REQUIRED SKILLS AND EXPERIENCE
<ul>
<li>Minimum of 2 years of experience in case management or direct service, preferably in supportive housing, domestic violence services, or homeless services.</li>
<li>Knowledge of OTDA regulations, ESSHI guidelines, and supportive housing best practices.</li>
<li>Experience working with survivors of domestic violence and families with complex needs.</li>
<li>Strong crisis intervention, de-escalation, and safety planning skills.</li>
<li>Ability to work with diverse populations including LGBTQIA+ individuals, immigrants, older adults, and individuals with mental health or substance use.</li>
<li>Strong organizational, writing, and time management skills.</li>
<li>Ability to manage multiple priorities in a fast-paced environment.</li>
<li>Proficiency in Microsoft Office and data management systems.</li>

POSITION OVERVIEW

The Case Manager is responsible for direct service provision to residents. Direct services include individual counseling, group counseling, advocacy services, referrals and follow-up, home management and financial counseling. Individual must demonstrate sensitivity to issues related homelessness. In addition, the Case Manager will make all necessary linkages to community agencies to provide proper placement of residents in areas of permanent housing, referral to engage in after care services prior to leaving the agency, vocational and educational services, and to assist families in securing public assistance, Medicaid or other benefits.

MAJOR DUTIES AND RESPONSIBILITIES
<ul>
<li>Maintain caseload of 25-30 families.</li>
<li>Conduct intake interviews, as assigned, to assess and evaluate residents for admission using DHS CARES system and as outlined by URI</li>
<li>Engage residents in initial and ongoing treatment processes, through individual and group counseling, under clinical supervision of the Social Work Supervisor.</li>
<li>Conduct independent living plans on at least a biweekly basis in DHS CARES system</li>
<li>Monitor case plans for effectiveness and individual client needs, functional capabilities and assistance needed to increase independent functioning</li>
<li>Develop and maintain housing referral resources, assist residents in the process of locating and securing permanent housing, in conjunction with Housing Specialist.</li>
<li>Assists all residents in the process of referral and placement in community based resources including after care services</li>
<li>Assess residents&rsquo; vocational and educational needs, aptitudes and interests through interviews and testing. Develop and maintain placement and follow-up as appropriate after assessment and counseling.</li>
<li>Develop and maintain liaison activities with DHS, HRA. Public Assistance and Medicaid Administrators and Workers, to assist eligible residents in securing Public Assistance and Medicaid by acting as a personal advocate through telephone or personal visit/contact</li>
<li>Assist in the development of savings plans with clients as applicable</li>
<li>Assist in the performance of weekly unit inspections of all units, including child welfare-involved families with children from birth to six(6) months of age and all other units on a biweekly basis</li>
<li>Explore where possible, family reunification as an option for transitioning out of shelter</li>
<li>Regularly assess the safety and well-being of clients</li>
<li>Make appointments for residents&rsquo; physical exams.</li>
<li>Develop with the resident, a contract of mutually agreed goals and objectives to be accomplished within an agreed upon timetable.</li>
<li>Facilitate psycho educational groups in conjunction with direct service staff.</li>
<li>Assist in the coordination of on-site and off-site services</li>
<li>Participate in training seminars conducted to enhance and develop clinical skills with the client population.</li>
<li>Attend weekly individual case supervision, group supervision, monthly case conferences, and staff meetings.</li>
<li>Function independently in time of facility coverage, including, responsibility for appropriate action in crisis situations per agency procedures.</li>
<li>Perform other duties as assigned by the Social Work Supervisor.</li>
</ul>
SPECIFICATIONS FOR EDUCATION/CERTIFICATIONS/LICENSES

<li>Bachelors Degree in Human Services.</li>
</ul>
REQUIRED SKILLS AND EXPERIENCE
<ul>
<li>Bilingual candidates are strongly encouraged to apply.</li>
<li>Two (2) years of counseling experience, preferred, in a residential shelter.</li>
<li>Three (3) years of crisis counseling experience preferred in a residential shelter for homeless individuals and/or families.</li>
<li>In addition, must have training and experience in assessing the vocational and educational needs and aptitudes of clients, thorough familiarity and success with engaging clients experience assisting eligible clients through the process of obtaining Public Assistance, Medicaid and other benefits.</li>
<li>Experience with supportive group facilitation.</li>
<li>Demonstrated sensitivity and experience with and/or knowledge of homelessness within disenfranchised communities including individuals whom identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, the elderly and individuals with substance abuse and/or mental health issues.</li>
<li>Familiarity with DHS regulations and DHS Cares Reporting.</li>
<li>Must possess a demonstrated ability in delivering client services including: communication and listening skills, understanding of human interaction and personality development, the ability to write and record case documentation, and demonstrated skills in group counseling.</li>
<li>Employee must be computer-literate and must have knowledge of Microsoft Office Applications.</li>
<li>Must have ability to be flexible of schedule as needed.</li>
