<p></p><div><div><div><b>Job Summary: </b>The emphasis of the position is the handle day-to-day patient care within the setting of a primary care health center under the supervision of an RN/Clinical Coordinator and provider.</div><div></div><div><b><br>
Job Duties:</b></div><div></div><div><br>
Under Supervision of Physician/Midlevel/Nurse</div><ul><li>
Initial triage of patients regarding urgency of presenting problems.</li><li>
Locate patient in examination room and make available to the provider all medical records of the patient and any supplies or instruments that may be required by the provider.</li><li>
Collect and record complete first order data (history and basic vital signs) before a patient is presented to the provider.</li><li>
Assist provider as required in treatment or examination of patient.</li><li>
Perform authorized lab tests and record information in designated files.</li><li>
Perform EKG tests as required.</li><li>
Keep all rooms supplied to needs of provider and staff.</li><li>
Check rooms for cleanliness after each patient visit.</li><li>
Maintain check on health center supplies and list items to be purchased.</li><li>
Schedule diagnostic test for patients as per provider&rsquo;s order as needed.</li><li>
Provide instruction to patients in areas of medications, diet, health maintenance and diagnostic tests.</li><li>
Record and observe provider orders.</li><li>
Prepare specimens for outside lab test.</li><li>
Make notations in EHR.</li><li>
Ensure patient comfort and well-being while at health center.</li><li>
Assist provider with procedures as needed.</li><li>
Administer medications to patients according to provider orders.</li><li>
Prepare sterile materials for use as needed.</li><li>
Dispose of expendable material according to sanitation guidelines.</li><li>
Perform clinical tasks such as dressing changes and ear irrigation as ordered by provider.</li><li>
Assist in care coordination which may include obtaining test and referral results and communication with various entities including but not limited to community organizations, health plans, facilities and specialists.</li><li>
Proactively address needs of the patients and families through the concept of population management.</li><li>
Effectively communicate with all patients with specific attention to the vulnerable patient population.</li><li>
Provide self-management support in the form of patient coaching and motivational interviewing when appropriate.</li><li>
Actively participate in the medical home care team which is comprised of clinical and non-clinical staff working towards a common goal: delivering quality care to our patients.</li><li>
Acknowledges the importance of a team approach when providing patient care.</li><li>
Communicate and coordinate effectively with all members of the care team.</li><li>
Maintain compliance with the organization&rsquo;s confidentiality policy in accordance to the Health Insurance Portability and Accountability Act (HIPAA).</li><li>
Perform other duties as assigned.</li></ul><div></div><div><b><br>
Working Conditions:</b></div><ul><li>
Position requires manual dexterity for operating standard office equipment.</li><li>
Hearing must be within range of normal human conversation.</li><li>
Position requires the ability to maintain concentration and attention to detail for long periods of time in order to maintain accurate records pertaining to patient and/or account information.</li><li>
Position functions within a normal office environment.</li></ul><div></div><div><b><br>
Qualifications:</b>.
</div><ul><li>Graduate of Medical Assistant Degree.</li><li>
Certification or equivalent experience.</li><li>
Good organization and ability to multitask.</li><li>
Friendly personality with the desire to work with the public.</li></ul></div><br>
