Job Summary
 
The RN-Clinical Coordinator is responsible for coordinating multidisciplinary care across various departments, including ICU, Medical, Surgical and Ancillary Units, ensuring quality patient care delivery. The role involves collaborating with nursing staff, physicians, and ancillary departments to assess, plan, implement, and evaluate patient care while supporting quality improvement initiatives and regulatory compliance in an acute care hospital setting.
 
Essential Functions
 
Coordinate patient care across ICU, Medical, and Surgical Units to ensure optimal patient outcomes and continuity of care.
 Collaborate with interdisciplinary teams to assess, plan, implement, and evaluate patient care plans.
 Provide clinical guidance to nursing staff, serving as a resource for clinical procedures, protocols, and hospital policies.
 Participate in quality improvement initiatives, monitoring patient outcomes and implementing corrective actions as needed.
 Conduct daily patient rounding to assess patient needs, monitor care quality, and provide support to nursing staff.
 Facilitate communication between medical staff, patients, families, and ancillary departments to ensure effective care coordination.
 Assist in discharge planning, ensuring patients receive appropriate follow-up care instructions and resources.
 Develop and implement educational programs to enhance clinical competencies and promote adherence to best practices.
 Monitor compliance with regulatory guidelines, including Joint Commission, EMTALA, and hospital-specific protocols.
 Document patient care activities accurately in the electronic medical record (EMR) and other clinical systems.
 Perform other duties as assigned by the Quality Director or Chief Nursing Officer.

Education
 
Associate Degree in Nursing (ADN) required.
 Bachelor of Science in Nursing (BSN) or Master’s in Nursing, Healthcare Management, or Administration preferred.
 
Licenses/Certifications
 
Current RN licensure in the state of practice or compact state required.
 Basic Life Support (BLS) certification required within timeframe specified in facility policy.
 
Experience
 
Minimum of 2 years of acute care inpatient nursing experience required.
 Experience in patient care coordination, quality improvement, and leadership preferred.

Job Summary
The Clinic Manager is responsible for overseeing the daily operations of hospital-owned physician practices, ensuring high-quality patient care, operational efficiency, and regulatory compliance. The role involves managing staff, developing and monitoring budgets, implementing practice policies, and fostering effective communication between clinical and administrative staff. The Clinic Manager also collaborates with senior leadership to develop strategic initiatives aimed at optimizing service delivery and patient satisfaction.

Performs patient observation, documents patient care and response to care and communicates significant information to the registered nurse and other appropriate team members.
Essential Functions<ul><li>
Assists patients with activities of daily living</li><li>
Consults with and keeps supervisor informed of unit activities, needs, and problems related to patient care</li><li>
Provide basic care to ill patients</li><li>
Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, pre operative and post operative care, and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift</li><li>
Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner.</li><li>
Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room</li><li>
Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting</li><li>
Assists in patient admission, transfer, and discharge procedures</li><li>
Assist with handling and care of patient belongings and other personal property</li><li>
Answers patient call lights, telephone, and paging system and responds as appropriate</li><li>
Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold</li><li>
Able to release, remove, and reapply restraints to patient under the direction of an RN</li><li>
