RN Quality &amp; Clinical Coordinator (Full-Time) Reports To: Clinical DirectorPosition SummaryVisiting Nurse Homecare and Hospice of Carroll County seeks a detail-oriented RN Quality &amp; Clinical Coordinator to ensure clinical excellence and quality across home health and hospice services. This office-based role combines chart audits, telephone triage, provider coordination, and staff mentoring to promote high-quality care and documentation readiness. The ideal candidate is a proactive RN with homecare experience who thrives in a collaborative nonprofit environment.Key ResponsibilitiesQuality Assurance &amp; Chart Review<ul><li>Conduct routine clinical chart audits and documentation reviews for home health and hospice records.</li><li>Monitor adherence to state/federal regulations, and agency policies.</li><li>Identify documentation gaps and provide staff education/feedback.</li><li>Support survey readiness and track corrective actions for quality concerns.</li></ul>Clinical Coordination &amp; Office Support<ul><li>Serve as clinical resource for field nurses, providing guidance on care, documentation, and regulations.</li><li>Perform telephone triage for patient/caregiver calls and coordinate with providers for orders and updates.</li><li>Facilitate communication between clinicians, physicians, and patients to ensure timely care coordination.</li><li>Assist intake/scheduling teams with clinical appropriateness questions.</li></ul>Documentation Oversight<ul><li>Review patient records for accuracy, timeliness, regulatory compliance, and clinical appropriateness.</li><li>Monitor completion of assessments, plans of care, visit notes, and certifications.</li><li>Reinforce documentation best practices through staff education.</li></ul>Staff Education &amp; Support<ul><li>Mentor nurses on regulatory updates, documentation standards, and clinical best practices.</li><li>Participate in QAPI activities and support agency policy implementation.</li></ul>QualificationsRequired<ul><li>Graduate of accredited nursing program; BSN preferred.</li><li>Current, active RN license in New Hampshire.</li><li>3&ndash;5 years nursing experience, with home health, hospice, or community care preferred.</li><li>Strong clinical documentation and regulatory knowledge (CMS CoPs familiarity).</li><li>Excellent communication, problem-solving, and organizational skills.</li><li>Ability to manage multiple priorities in a small agency setting.</li></ul>Preferred<ul><li>Experience with clinical audits, quality assurance, or compliance oversight.</li><li>Proven ability to educate and mentor clinical staff.</li><li>Familiarity with KanTime or similar homecare EMR systems.</li></ul>Job Types: Full-time, PermanentBenefits:<ul><li>Dental insurance</li><li>Flexible spending account</li><li>Health insurance</li><li>Paid time off</li><li>Retirement plan</li></ul>Work Location: In person

This position plays a key role in ensuring smooth executive operations, supporting leadership initiatives, and maintaining exceptional confidentiality and professionalism. The ideal candidate is proactive, tech-savvy, mission-driven, and capable of managing multiple priorities with discretion and accuracy.Essential FunctionsExecutive &amp; Leadership Support<ul><li>Serve as the primary administrative support to the Executive Director.</li><li>Provide administrative and project-based support to members of the Leadership Team.</li><li>Prepare Board meeting materials, including agendas, dashboards, and packets; take and distribute meeting minutes as assigned.</li><li>Manage document preparation, correspondence, scheduling, and travel coordination.</li></ul>Donor Database &amp; Systems Management<ul><li>Administer the agency&rsquo;s donor database, ensuring accurate, timely, and secure donor and gift records.</li><li>Maintain data integrity through updates, routine clean-up, reports, and mailing lists.</li><li>Support donor acknowledgements with reports, lists, and mail merges.</li><li>Partner with Finance on donor-related reporting and reconciliations to align database and financial records.</li></ul>Operations &amp; Communication<ul><li>Assist with data entry and maintenance across administrative systems as needed.</li><li>Support process improvement and documentation initiatives that strengthen agency efficiency.</li><li>Coordinate internal communications and provide administrative support for meetings.</li><li>Provide cross-coverage for administrative colleagues as needed.</li></ul>Confidentiality &amp; Professional Standards<ul><li>Uphold the highest standards of confidentiality, professionalism, and discretion in all interactions.</li><li>Adhere to all agency policies, including HIPAA and employee handbook standards.</li><li>Represent the Executive Director and agency with professionalism in all internal and external communications.</li><li>Support a respectful, mission-driven workplace culture.</li></ul>Other Duties<ul><li>Perform related duties as assigned to support agency operations and mission fulfillment.</li></ul>QualificationsRequired<ul><li>2+ years administrative or executive support experience (healthcare or nonprofit preferred).</li><li>Proven professionalism, integrity, and discretion with confidential information.</li><li>Strong organization, communication, and writing/proofreading skills.</li><li>Excellent interpersonal skills and a service-oriented approach.</li><li>High attention to detail and accuracy with data and reports.</li><li>Sound judgment and problem-solving skills.</li><li>Proficiency with Microsoft Office or Google Workspace and ability to learn new systems.</li></ul>Preferred<ul><li>Experience supporting executives or a Board.</li><li>Experience managing a donor database or similar system.</li><li>Background in home health or hospice agency or other non-profit organizations.</li></ul>Job Type: Full-timeBenefits:<ul><li>401(k)</li><li>Dental insurance</li><li>Health insurance</li><li>Paid time off</li></ul>Work Location: In person
