Coordinator Continuous Care Clinical Training

<div><p>The Caseworker is that member of patient care operations whose primary function is to assist in providing direct casework services to the patient and family within the hospice program of care.</p>
QUALIFICATIONS
<ul><li>Qualified candidates will possess a minimum of two (2) years experience in a health-care environment in which the primary job function was community resources utilization which included patient contact.</li>
<li>Reliable transportation with adequate insurance coverage for driver and passenger or requires ability to cope with the stress of experiencing repeated loss.</li>
<li>Extensive knowledge of community resources within the specific community in which the hospice is located.</li>
<li>Knowledge of local, county, applicable state and federal assistance programs.</li>
<li>Capacity to work with minimal supervision.</li>
<li>Ability to become proficient in company software programs.</li>
</ul>EDUCATION
<ul><li>Bachelor's Degree in Social Work or a related Social Science field from an accredited school.</li>

</div><div></div><div><div>As a home care hospice physician, you will use your compassion, communication skills and deep expertise to build meaningful relationships with patients and families, providing the highest-quality care at the most challenging point in life. You will see the person behind each prognosis and create care plans centered around the patient&rsquo;s wishes, needs, and beliefs, often working in concert with the patient&rsquo;s primary care physician.</div></div><div><br>
</div><div></div><div><div>Manage approximately 45-60 patients in their home settings via an interdisciplinary team approach. Make visits in-person and via telehealth. Participate in Weekly team IDG meetings.</div></div><div><br>
</div><div></div><div><div>With your strong sense of commitment, you will advocate for your patients, discussing goals for their care while addressing their hopes and fears. You will live the hospice mission, seeking teachable moments everywhere, and educate patients, caregivers, and the communities you serve on the benefits of compassionate, dignified end-of-life care.</div></div><div><br>
</div><div></div><div><div><b>WHERE YOU&rsquo;LL WORK</b></div></div><div><br>
</div><div></div><div><div>Each day is different. You will spend most of your time caring for hospice patients and families wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility.</div></div><div><br>
</div><div></div><div><div>Once a week, you&rsquo;ll visit your local program office to discuss cases with members of your interdisciplinary team, including social workers, chaplains, and nurses. All of these roles collaborate to provide holistic support for the patients and families we serve.</div></div><div><br>
</div><div></div><div><div><b>WHAT&rsquo;S EXPECTED FROM YOU</b></div></div><div><br>
</div><p></p><div><p>VITAS physicians must be licensed to practice medicine or osteopathy in the state in which their program operates. In addition to being board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty, you should be familiar with the principles of hospice and registered with the DEA.</p>
