</b></h2><p></p><p>The Bachelor&rsquo;s prepared social worker provides the professional social work component of the Care Management services. Care Management in Hospital/Health Care Systems is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Care Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Care Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self-determination, competency and cross coverage will be accomplished by collaborating with case management. Case Management will provide oversight by MSW if needed.
</p><p></p><p><b>JOB QUALIFICATIONS
</b></p><p></p><p><b>Education: </b>Bachelor&rsquo;s degree of Social Work or related field.
</p><p></p><p><b>Licensure: </b>Exempt from Georgia Licensure
</p><p></p><p><b>Experience: </b>Prefer experience in acute care setting or post-acute inpatient setting.
</p><p></p><p><b>Skills: </b>The Bachelors prepared Social Worker must be a highly motivated, self-directed individual with a demonstrated understanding of acute care Care management issues. The individual will have excellent verbal and written communication skills, strong listening skills, critical thinking and analytical skills, ability to build positive working relationships with the interdisciplinary team, ability to communicate with multiple levels in the organization (e.g. managers, physicians, clinical and support), government agencies and post-acute providers. He or she must have the ability to obtain and interpret information in terms of the patients&rsquo; needs that are age appropriate, and the ability to advocate/negotiate patient/family assessed needs with Durable Medical Equipment / Home Health Care agencies, Nursing Facilities, and/or insurance companies <b>.</b></p></div>
