Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC), a federally qualified health center in Monmouth County, provides accessible, high‑quality healthcare through preventive and primary care, wellness services, mental health counseling, prenatal care, and more across our four community‑focused locations. We are seeking a Behavioral Health Consultant / Licensed Clinical Social Worker (LCSW) to join our integrated behavioral health program at the Keyport Primary Care Center, where you&rsquo;ll help expand whole‑person, coordinated care for the Bayshore community as part of a collaborative and innovative team, with occasional rotation to other CHC sites as needed. 
 
About the Role
You&rsquo;ll provide behavioral health consultation and brief intervention services for children, adolescents, adults, and families within a fast‑paced primary care setting. Working alongside medical providers, peer support specialists, and care managers, you&rsquo;ll help patients address mental health needs, substance use concerns, and psychosocial challenges. Our integrated model includes primary care, dental services, on‑site lab work, MAT, SUD treatment, and care coordination&mdash;all under one roof. The position requires travel across CHC locations&mdash;including Asbury Park, Red Bank, Keyport, and Freehold&mdash;to ensure consistent, team‑based behavioral health support throughout the system. 
What You&rsquo;ll Do<ul><li>
Provide scheduled and same‑day behavioral health consults in collaboration with primary care</li><li>
Conduct assessments, brief interventions, and crisis evaluations</li><li>
Deliver services both in person and via telehealth</li><li>
Offer psychoeducation in a patient‑centered, accessible way</li><li>
Maintain accurate documentation in the Electronic Health Record and follow all billing/coding requirements</li><li>
Identify at‑risk patients through screenings, warm handoffs, and referrals</li><li>
Collaborate within a Patient‑Centered Medical Home model and work effectively with diverse care teams</li><li>
Participate in meetings and activities supporting the integrated care model</li><li>
Contribute to program evaluation and quality improvement efforts</li><li>
Ensure compliance with organizational policies, state regulations, and payer requirement</li></ul> 
What You Bring<ul><li>
Master&rsquo;s Degree in Social Work and active New Jersey LCSW</li><li>
At least one year of supervised clinical experience (community health preferred)</li><li>
Strong verbal and written communication skills</li><li>
Excellent interpersonal skills and comfort working with diverse populations</li><li>
Ability to collaborate within multidisciplinary teams</li><li>
Eligibility to participate with all public and private insurance payers</li><li>
Bilingual Spanish‑speaking strongly preferred</li></ul>

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits. The VPS is comprised of physicians, nurse practitioners and physician assistants with the mission of treating elderly, frail, homebound patients with dignity and compassion .
Position Summary:
Responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families 
Principal Responsibilities:<ul><li>
Supports organization&rsquo;s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers</li><li>
Assesses, diagnoses and develops appropriate plan of care to treat homebound patients</li><li>
Monitors the patient&rsquo;s condition and responsiveness to the treatment provided</li><li>
Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations</li><li>
Participates in organizational wide QAPI program and attends agency sponsored in-service programs</li><li>
Adheres to agency infection control and safety policies</li><li>
Adheres to the organization&rsquo;s policy in regards to absenteeism and appearance</li></ul> 
Job Competencies &amp; Minimum Qualifications:<ul><li>
Master&rsquo;s degree in Nursing required from an NLN accredited program</li><li>
Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA</li><li>
National certification as an Adult or Geriatric Nurse Practitioner</li><li>
2 + years of APN experience and/or significant experience as a registered nurse</li><li>
Excellent communication, organizational and interpersonal skills</li><li>
Strong computer skills</li></ul>
