POSITION SUMMARY
As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide skilled therapy services in the home, facility, or educational environment. Caseloads may include adult and/or pediatric patients depending on experience and interest.
Responsibilities include:<ul><li>
Completing PT and OASIS assessments as appropriate</li><li>
Developing and implementing individualized plans of care</li><li>
Providing therapeutic interventions and caregiver education</li><li>
Monitoring patient progress and adjusting treatment plans as needed</li><li>
Collaborating with interdisciplinary team members</li><li>
Supervising PTAs, students, and home care aides when applicable</li><li>
Maintaining timely and accurate clinical documentation
</li></ul>QUALIFICATIONS<ul><li>
Degree in Physical Therapy from an accredited program</li><li>
Current Vermont PT license</li><li>
Valid driver's license, ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.</li><li>
Home health or prior clinical experience preferred</li></ul>
