<div>CLASSIFCATION SUMMARY:
Volunteers of America Southwest&rsquo;s (VOASW) Enhanced Care Management (ECM) program provides a whole person approach to care for Medi-Cal beneficiaries. VOASW believes in individualizing services based on need and what works best for the member, thus through care management and care coordination we asses need and connect individuals to services and resources most appropriate for them. Through the delivery of these services, we hope every member can improve their quality of life, establish a support system and develop healthy living choices. The Supervising Care Manager ensures effective and efficient care management and care coordination throughout the Enhanced Care Management program by carrying a caseload and providing direct supervision to Care Managers. The role serves as a supervisory position that is responsible for direct services and the direct oversight of staff. The Supervising Care Manager assists staff in making informed decisions about member care and services while ensuring staff have knowledge about resources.
ESSENTIAL DUTIES AND RESPONSIBILITES:
Client caseload management:<ul><li>
Conduct comprehensive and individualized risk assessment, care planning, and reassessment.</li><li>
Provides strategic and individualized care coordination services.</li><li>
Maintain ongoing communication with members through care management.</li><li>
Coordinates referrals with ECM team and external partners</li><li>
Tracks member progress towards identified goals and program outcomes.</li><li>
Ensures error free documentation that meets program requirements.</li><li>
Provides day-to day support and supervision to Care Managers</li><li>
Reviews member files and provides advice, direction, and support as it relates to documentation requirements.</li></ul>

EDUCATION AND EXPERIENCE:
<ul><li>BA, LMSW/LCSW/LMHC or RN/LPN degree</li><li>
2 years minimum of case management experience</li><li>
2 Years minimum of management and/or supervisory experience of high-volume outpatient client service</li><li>
Demonstrated skill and knowledge in client, engagement, community network building, and quality assurance documentation/records</li><li>
Strong understanding of cultural competency with the target population</li><li>
Medi-Cal experience preferred</li><li>

Supervising Care Manager

The Care Manager is the primary point of contact for Medi-Cal beneficiaries and directly provides care management and care coordination services. Care Managers are responsible for completing comprehensive assessments, establishing care plan, coordinating services and referrals and providing education and support to help members make healthy lifestyle choices. Care Managers facilitate conversations between interdisciplinary Care Teams (including Community Health Workers, Care Coordinators, Lead Care Managers, primary care physicians, and other providers delivering care to the member.)
ESSENTIAL DUTIES AND RESPONSIBILITES:
Client caseload management:
<ul><li>Conduct assessments and reassessments, care planning, and care management.</li><li>
Schedule in-person/home visits and electronic check-ins with members.</li></ul><ul><li>
Maintain extensive knowledge of local community support services, inducing but not limited to health care, mental health care, housing, basic needs, etc.</li><li>
Coordinate care and services with external care providers including PCPs, Specialty Physicians, Nurses, Psychologists, Psychiatrists, etc.</li><li>
Document interactions with member including direct and indirect services.</li><li>
Provide ongoing support and education to members regarding health lifestyle choices.</li><li>
Regularly review and update care plan.</li></ul><ul><li>
Work with Medi-Cal and community providers to coordinate services for members.</li><li>
Provide members with education and services to prevent ER visits, hospitalizations and readmissions.</li></ul>
Organizational and administrative duties:
<ul><li>Participate in Care Team meetings and Care Conferences.</li><li>
Document client services EHR. Track all services provided to member including direct and indirect services and time spent on each activity.</li></ul><ul><li>
Establish and retain client referral systems from care coordination systems.</li><li>
Complete reporting requirements according to organization standards.</li><li>
Track client information, schedules, files, and forms in a confidential manner.</li><li>
Maintain ongoing knowledge of program requirements, expectations and services.</li><li>
Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor.</li><li>
Participate quality assurance and monitoring activities for service delivery and documentation.</li></ul>

JOB SUMMARY AND PURPOSE
The Custodian performs daily janitorial duties, and immediate minor repairs to the facility, reporting any maintenance or safety deficiency to management for correction. Performs all tasks needed to maintain cleanliness, sanitation and safety of the facility, adhering to janitorial schedules provided
DUTIES AND RESPONSIBILITIES
<ul><li>Maintains the interior and exterior of the facility, including building, parking lot and grounds, in good condition
<ul><li>Daily cleaning of assigned areas using various cleaning products.</li>
<li>Restock bathrooms</li>
<li>Responsible for dusting, window cleaning, waste cans, and vacuuming lobby area and hallways.</li>
</ul></li><li>Performs or assists in minor repairs that might include the following: Carpentry, Plumbing, Interior painting, Refurbishing and cleaning, Landscape and irrigation systems.</li>
<li>Follows daily checklist provided by supervisor; Assures tools and supplies are stocked and appropriately secured in store rooms; maintains cleanliness and order of store rooms</li>
<li>Checks facility daily for any safety concerns; understands local and building safety codes and requirements as well as OSHA, health department and EPA requirements; reports all deficiencies to management</li>
<li>Prepares and sets-up rooms for meetings, classes and events</li>
<li>Other duties as assigned</li>
</ul>
ESSENTIAL DUTIES:

Qualifications
REQUIREMENTS:

EXPERIENCE:
<ul><li>Experience cleaning high foot traffic areas and bathrooms, or similar building maintenance experience</li>
</ul>
PREFERRED QUALIFICATIONS:
<ul><li>High school diploma (or GED/High School Equivalence Certificate)</li>
<li>Custodial or building maintenance experience in a shelter or a school setting preferred</li>
<li>Ability to read and write instructions in English preferred</li>
<li>Background in construction trades a plus</li>
<li>Proven familiarity with homelessness and related issues a plus</li>
