JOB SUMMARY AND PURPOSE
Case Manager Entry provides entry level case management services to clients classified as low acuity, serving on a team that is the central point of contact to coordinate services and care for assigned clients. These positions serve individuals and families residing in interim and permanent housing. They support and provide services in the areas of assessment; care planning; coaching and social support; connections to direct services; care coordination and system navigation; education about the health and social service system; outreach and engagement, and capacity building for individuals who were experiencing homelessness and are now mostly housed.
DUTIES AND RESPONSIBILITIES

Qualifications

Case Manager Entry

<div>Reporting to the Assistant Director, Resident Services, the Accountability Specialist (ACS) primary responsibilities include coordinating resident approved movement schedule with case managers, monitor resident on GPS devices assigned to Home Confinement and job site. ACS will install GPS on resident monitor resident movements while on home confinement, social pass, treatment, employment, or other authorized community activities to ensure compliance with the Bureau of Prisons Community Re-entry guidelines. Monitoring shall include unannounced visits to residents homes, pass locations and places of employment; telephone verification of residents scheduled locations; and appropriate documentation of contacts in the resident file and other tracking records. The ACS provides guidance to the facility on resident whereabouts outside the facility. ACS coordinates monitoring activities, document any violation, master tamper alarm, conduct investigation. The employee will report to the Assistant Director Resident Services.
RESPONSIBILITIES:<ul><li>
Install GPS devices on resident and monitor residents movement, home confinement, job , treatment site, etc.</li><li>
Coordinate with case managers to and develop implementation resident schedule to ensure compliance with Bureau of Prisons (BOP) and America Corrections Association (ACA) guidelines, in home confinement, pass, employment, or other authorized community activity.
</li><li>Coordinate job-site visits with the Employment Placement Specialist, ensuring that the schedule of visits is appropriate and reasonable.
</li><li>Implement community-based monitoring based on the approved monitoring plan, and as mandated by ACA, VOACC and the funding source.
</li><li>Conduct home detention site visits according to a standard checklist, and coordinate with Control Center staff to verify each residents authorized located and schedule.
</li><li>Maintain accurate monitoring inventory of GPS devices, records site visits, home confinement, telephone contacts, GPS tracking alerts and incidents and monitoring documentation weekly and ensure they are properly filed in the Secure-Manage (SM) under each resident log</li><li>
Report any deviation from residents authorized community activities to the Program Director/designee, and complete disciplinary incident reports accordingly.
</li><li>Coordinate and conduct initial meeting with the resident and their family members to complete orientations to home plans and visits to the residents release address.</li><li>
Maintains and demonstrate working knowledge of ACA requirements, VOACC policies and procedures and operational procedures, as well as, contractual obligations established by the funding source.</li><li>
Perform functions related to the Center Disciplinary Procedures as directed.</li><li>
Drive on company business to perform residential and employment site visits and transport clients as required.</li><li>
Participate in Program Review Team Meetings weekly and attend staff meetings and other VOACC work-related meetings and functions as directed.</li><li>
Participate in the provision of pre-service and in-service training for staff as directed.</li><li>
Prepare weekly, monthly or other periodic reports as required in the performance of position duties.</li><li>
Participate in 44 hours annual training as mandated by the ACA, VOACC, and funding source, and maintain certification in CPR/First Aid.</li><li>
Represent the program to the public, community agencies, and funding sources, professionally and appropriately.

JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) provides intensive case management services to eligible clients, meeting participants at least weekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available.

JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) &ndash; Problem Solving Specialist provides intensive housing crisis response and case management services to eligible clients, meeting participants at least weekly. Using active listening techniques, the Case Manager &ndash; Problem Solving Specialist is the primary point of contact to coordinate creative resolutions to a housing crisis &ndash; locating both temporary and permanent housing. Located at a VOALA shelter, and working is the field as needed, the Case Manager (Specialist) &ndash; Problem Solving Specialist focuses on resolving the housing crisis, but can refer to other resources as needed.
DUTIES AND RESPONSIBILITIES
<ul><li>As a member of the program case management team, provides information referrals, crisis intervention, and assistance with permanent housing placement for participants</li>
<li>Assists case managers in conducting the culturally appropriate, comprehensive screenings and assessments to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a Housing Stability Plan</li>

Case Manager Problem Solving Specialist

We engage faith, relationship-building, and volunteers wherever we go to lift up and support Colorado&rsquo;s most vulnerable citizens.
** The Shelter Coordinator position provides &ldquo;rotating 30 minutes of break time coverage&rdquo; to the Shelter program
staff in the Denver Metro Area Shelters. This is a rotating position where the Shelter Coordinator will travel to
assigned sites to relieve staff for their 30-minute break. The Shelter Coordinator position is responsible for
managing the facility (in the time they are covering). The programs under Shelter purview serve multiple
populations in different programs: a Safe Haven for chronically homeless women with mental health concerns, a
family shelter, a Grant Per Diem program for Veteran Families and unaccompanied individuals, and a shelter for
older women. Service modalities for all programs under RYES include Housing First Trauma Informed Care, Harm
Reduction, Motivational Interviewing, and Crisis De-escalation. Job duties include supporting and coordinating with
other staff in the delivery of appropriate housing stability and support services for individuals and families
experiencing homelessness, including ensuring participants needs are met, maintaining the safety and wellbeing of
program participants, and maintaining the cleanliness of the facility. Further, the Shelter Coordinator will have a
basic understanding of mental health services and may have the opportunity to develop in specialty areas
including housing, employment, legal services, benefits, or others as indicated by the needs of the program(s).
Essential Duties and Responsibilities<ul><li>
Ensures that the program participants&rsquo; voices are heard, and when appropriate the participants ideas play a prominent role in programming.</li><li>
Responsible for possessing an in-depth knowledge regarding program requirements, including the referral process, support services, and field-relevant best practices.</li><li>
Demonstrates knowledge of participant-specific resources, a passion for serving the population and people experiencing homelessness, and the ability to contribute to the program-wide objectives of a diverse and dynamic team.</li><li>
Clearly documents all participant interactions along with required eligibility and demographic information. Can develop specialized knowledge related to service delivery database systems within the organization including Service Point and Homeless Management Information System (HMIS).</li><li>
Performs professional duties by maintaining confidentiality of all information and participating effectively within and across teams.</li><li>
Maintains physical and emotional safety of all program participants in the facility. Manages the milieu and addresses any program participant conflicts immediately to de-escalate. Will contact emergency personnel as necessary and will accurately and adequately complete any reports or documentation required.</li><li>
Supports team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field.</li><li>
Oversees and participates in general program facility maintenance such as assisting in cooking, cleaning, supplying linen and personal items, conducting building walk-throughs, enforcing rules, checking chore completion, setting and checking alarms, etc.</li><li><ul><li>Assists in the training and supervision of volunteers during shifts worked. Ensures proper volunteer tracking documentation and indemnity forms are completed.</li></ul></li><li>
Maintains regular front desk duties that include answering phone calls, taking messages, implementing participant intakes, accepting donations, handling mail, handing out bus pass and/or bus tickets, if applicable to the program.</li><li>
Serves as a mentor to junior personnel to develop knowledge and experience regarding best practice standards within their workgroup. May be responsible for training staff members assigned to the shift being covered.</li><li>

Working Conditions and Physical Requirements<ul><li>Ability to be single staffed while in the facility while having the responsibility for the safety of program participants.</li><li>Ability to quickly and safely go up and down stairs regularly throughout the shift to do perimeter checks, and in cases of an emergency.</li><li>Some lifting is required but should rarely exceed 50lbs.</li></ul>
Position Type and Expected Hours of Work<ul><li>This is a permanent - part time position</li><li>While Shelter Support Coordinators are on duty, they are likely single-staffed and unless they have established coverage for an approved reason, they will be expected to stay at the facility for the entirety of their shift.</li><li>All Shelter staff are considered &ldquo;essential staff,&rdquo; requiring to be on their scheduled shifts regardless of holidays or inclement weather. This is compensated as determined by HR.</li><li>All Shelter Support Coordinator staff must utilize the on-call protocol if unable to fulfill a shift.</li></ul>

Requirements:

Minimum Qualifications of Position<ul><li>
Associates degree in a related area or the equivalent experience in human services.</li><li>
One-year direct human service experience (on top of degree or human services experience equivalency).</li><li>
Good organizational and planning skills; works well as part of a team and independently.</li><li>
Complete program credentialing process, to include criminal and civil background checks within 30 days of hire and fingerprinting.</li><li>
Willing to obtain first aid, automated external defibrillator (AED) and cardiopulmonary resuscitation certifications and Narcan utilization training.</li><li>
Must possess a Colorado driver&rsquo;s license and state-mandated automobile insurance.</li><li><ul><li>Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).</li></ul></li></ul>
Preferred Qualifications of Position<ul><li>Experience working with individuals experiencing chronic homelessness, severe and persistent mental health concerns, and case management experience and/or shelter experience and/or milieu management.</li><li>Crisis intervention skills.</li></ul>
Knowledge and Skills<ul><li>Good written, oral, and interpersonal communication skills.</li><li>High level of competency working with Microsoft Office suite and cloud-based applications.</li><li>Strong time-management and prioritization skills.</li><li>Experience working both independently and, in a team-oriented, collaborative environment.</li><li>Strong organizational skills.</li></ul>
