POSITION SUMMARY:</div><div>
Under direction of the Program Manager and with independent professional responsibility, the Case Manager is responsible for the development of a case management plan to assist program participants. They will consult with staff in the utilization of other functions at a highly skilled level in such areas as assessment, individual counseling, group facilitation, employment services, community resource development and housing.</div><div></div><div> 
OBJECTIVES/ACTIVITIES:</div><div>
A. Responsible for performing daily work requirements to achieve established objectives of the department.</div><div>
1. Performs and provides a written assessment of program participant&rsquo;s needs and develops individualized service plans with short- and long-term goals including support the goal of housing. 
2. Evaluates intensive short- and long-term individualized service plans on a regular basis and revises goals as appropriate. 
3. Facilitates an array of employment focused groups designed to meet participant needs. 
4. Educates program participants on issues surrounding living skills, parenting, budgeting and money management, health and hygiene, housing, and drug and alcohol abuse. 
5. Develops and maintains comprehensive lists of community resources for use by program participants; refers program participants accordingly. Works closely with Employment and Housing Specialists. 
6. Establishes individualized budgets with program participants and assists program participants in monitoring savings and expenditures. 
7. Conducts in-home visits and provides support services as necessary or directed. 
8. Documents all contact with program participants; prepares all screenings, assessments, service plans, evaluations, and reporting records. 
9. Maintains case notes and other documents in the Homelessness Management Information System (HMIS). 
10. Coordinates incoming and outgoing participants&rsquo; referrals with other community agencies. 
11. Attends case conferencing and other meetings and training as directed.

EQUIVALENT EDUCATION AND EXPERIENCE:</div><div>
A Bachelor&rsquo;s Degree in Social Work or a related field is preferred. This position requires two years of successful experience in the delivery of human services or any combination of education, training and experience, which would provide the knowledge and ability to complete the duties outlined above. Knowledge of social casework objectives, principles, and methods, employment readiness, permanent/supportive housing, public welfare laws and the functions of public welfare agencies, principles of community organization and problems which call for the use of both public and private community resources, individual and group behavior and effects of social and economic forces upon individuals and families, principles and techniques of interviewing, computer skills/applications, principles of cultural competency and diversity, knowledge of challenges concerning dysfunctional families, domestic violence issues, problems unique to women, problems unique to families, disadvantaged and underserved populations, people of color, persons with disabilities, individuals with chemically issues, and the general homeless population. Demonstrate ability in record keeping, good written and verbal communication skills. Mather Community Campus, Singles Program - Case Manager

<div><h4 class="jobSectionHeader">Function/Scope:</h4>
The Supportive Services for Veteran Families (SSVF) Case Manager is responsible for providing support and services to participating veteran households. The Case Manager may assist in connecting the Volunteers of America Pennsylvania (VOAPA) SSVF program with local and professional agencies and staff to help generate community partnerships, client referrals, and community resources. This position assists with ensuring that the SSVF program operates and delivers services according to program guidelines established by the U.S. Department of Veterans Affairs (VA), as well as VA grant requirements, VOAPA policies and procedures, and federal, state, and local regulations.
<h4 class="jobSectionHeader">
Responsibilities include, but are not limited to:</h4><h4 class="jobSectionHeader">
A. Principle Duties:</h4>
1. Provide comprehensive case management services for clients in accordance with policies and procedures and contract/funding source requirements.
2. Maintain a caseload of active clients, consistent with monthly program goals.
3. Develop, establish, and maintain effective outreach services and positive working relationships with SSVF staff and local/professional community agencies and resources.
4. Perform weekly outreach at designated homeless shelters, screening both referred and walk-in potential clients for eligibility and intake.
5. Maintain consistent contact with clients in accordance with program and agency regulations, to include phone contact, email, in-home visitation, community visitation, and in-office visitation.
6. Perform basic counseling skills (e.g., motivational interviewing, strengths-based approach) while employing best practices.
7. Assist clients with achieving personal housing goals in accordance with an Individualized Housing Plan.
8. Update the Individualized Housing Plan monthly (or sooner) to reflect changes in client circumstances or goals.
9. Maintain client confidentiality at all times, using best practices and adhering to local, state, and federal guidelines.
10. Work collaboratively with community housing providers, veterans service providers, homelessness service providers, the VA, family service providers, employers, and other professionals to provide housing stability, and where needed, income stability.
11. Ensure that clients' and program records are generated and maintained in accordance with policies and procedures and contractual and regulatory requirements.
12. Compose accurate, timely case notes on each client interaction using established templates and procedures, and submit weekly to the Senior Case Manager for review and correction.
13. Manage continuous improvement of the case management process.

B. Operational Functions:</h4>
1. Act as a liaison between the program and funding/regulatory and government agencies.
2. Assist in developing, implementing, and maintaining program operating documents and forms as requested and/or required.
3. Develop, maintain, and/or manage reports, including client file notes, as required. Collect, maintain, and disseminate data on clients, services, and operations as required.
4. Ensure all applicable federal, state, and local government and contractual regulations and requirements are met or exceeded.
5. Maintain compliance with VOAPA policies and procedures.
6. Ensure that all duties are carried out in a timely and orderly fashion so that SSVF functions in the most efficient manner possible.<h4 class="jobSectionHeader">
C. Professional Development:</h4>
1. Routinely meet with supervisory staff for individual, face-to-face supervision to discuss issues related to job performance or implementation.
2. Participate in staff meetings in accordance with program and agency requirements.

Qualifications:</h4>
The candidate must have a bachelor&rsquo;s degree related to human services-related field or at least two years of experience in case management and counseling (preferably with veterans and/or homeless individuals and families). In addition, the candidate must have strong oral and written communication skills; proven organizational and time management skills; keen attention to detail; the ability to develop effective working relationships with clients, staff, and community partners; maintain a professional appearance; and have a sensitivity to vulnerable populations.

This position requires driving a personal vehicle. The successful candidate must have a valid driver&rsquo;s license with the State of Pennsylvania, possess vehicle liability coverage, and have maintained a good driving record. This also requires frequent travel throughout the program&rsquo;s multi-county serving area.

Working Conditions:</h4>
VOAPA&rsquo;s SSVF staff work in a non-stationary environment and frequently travel throughout the programs regional serving area. They also engage with a diverse public and may be subjected to interruptions throughout the day.  ESSENTIAL DUTIES AND RESPONSIBILITIES:<div>
 &bull; Performs all sales and leasing activities to achieve the community&rsquo;s revenue and occupancy goals by greeting and qualifying prospects, conducting community tours and showing apartments, processing applications, credit screening, and criminal background checks, preparing the lease and move-in package, and ensuring a smooth resident move-in and lease signing.

RNDC-Leasing Agent

<div>Reporting to the Assistant Director, Resident Services, the Accountability Specialist (ACS) primary responsibilities include coordinating resident approved movement schedule with case managers, monitor resident on GPS devices assigned to Home Confinement and job site. ACS will install GPS on resident monitor resident movements while on home confinement, social pass, treatment, employment, or other authorized community activities to ensure compliance with the Bureau of Prisons Community Re-entry guidelines. Monitoring shall include unannounced visits to residents homes, pass locations and places of employment; telephone verification of residents scheduled locations; and appropriate documentation of contacts in the resident file and other tracking records. The ACS provides guidance to the facility on resident whereabouts outside the facility. ACS coordinates monitoring activities, document any violation, master tamper alarm, conduct investigation. The employee will report to the Assistant Director Resident Services.
RESPONSIBILITIES:<ul><li>
Install GPS devices on resident and monitor residents movement, home confinement, job , treatment site, etc.</li><li>
Coordinate with case managers to and develop implementation resident schedule to ensure compliance with Bureau of Prisons (BOP) and America Corrections Association (ACA) guidelines, in home confinement, pass, employment, or other authorized community activity.
</li><li>Coordinate job-site visits with the Employment Placement Specialist, ensuring that the schedule of visits is appropriate and reasonable.

JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) provides intensive case management services to eligible clients, meeting participants at least weekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available.

Qualifications
REQUIREMENTS:
<ul><li>Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications</li>

EDUCATION:

JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) &ndash; Problem Solving Specialist provides intensive housing crisis response and case management services to eligible clients, meeting participants at least weekly. Using active listening techniques, the Case Manager &ndash; Problem Solving Specialist is the primary point of contact to coordinate creative resolutions to a housing crisis &ndash; locating both temporary and permanent housing. Located at a VOALA shelter, and working is the field as needed, the Case Manager (Specialist) &ndash; Problem Solving Specialist focuses on resolving the housing crisis, but can refer to other resources as needed.
DUTIES AND RESPONSIBILITIES
<ul><li>As a member of the program case management team, provides information referrals, crisis intervention, and assistance with permanent housing placement for participants</li>
<li>Assists case managers in conducting the culturally appropriate, comprehensive screenings and assessments to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a Housing Stability Plan</li>
<li>Meets with participants to learn about family dynamics, and cultural factors that may assist in finding housing inside the participant&rsquo;s network or family, as a last resort utilizing the resources of the Program to obtain housing, helps participants establish a plan to sustain housing permanently</li>
<li>Assists in budgeting with participants, to determine what level of assistance (if any) is needed to resolve housing crisis</li>
<li>Coordinates follow-up services with participants and case management team</li>
<li>Identifies and locates housing options suitable for homeless participants, including referral to VOALA shelters as appropriate.</li>
<li>Creates and maintains a housing database linking tenants to apartments</li>
<li>Communicates with and visits, families, outside service providers and landlords to familiarize them with the program and the unique needs of participants</li>
<li>Conducts habitability and lead-based paint inspections of housing options as needed</li>
<li>Works directly (One on One) with landlords on behalf of clients and program</li>
<li>Develops support systems to meet individual client's housing needs; coordinates and facilitates move-in activities, rental negotiations, and service referrals as needed</li>
<li>Facilitates the use of community resources to assist both tenants and landlords in the event of problems leading to potential eviction</li>
<li>Provide service linkages and support systems to ensure identified needs are met, coordinate with outside service providers.</li>
<li>Continuously works to identify and apply new, creative approaches and solutions for each participant/family</li>
