 We are currently seeking qualified candidates for the following position:
 
Position: Case Manager
 
The Case Manager is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. S/he must assure that clients receive appropriate, competent and professional information and advice, referrals and advocacy services to achieve successful outcomes. May include supervision of direct care staff.
 
Horizon Program, comprised of scattered-site apartments located in the Bronx, enables single adults and families to live independently within in the community, as self-sufficiently as possible. Minimum Qualifications: 
<ul>
<li>Associate's degree or 60 college credits courses in Human Services field, or High School Diploma or GED with 2 years of relevant shelter experience.</li>
<li>The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.</li>
</ul>
Case Manager Principal Responsibilities: 
1. Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
 2. Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
 3. Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
 4. Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.

Position: Benefits Specialist
 
The Benefits Specialist offers support to eligible formerly homeless clients living in Permanent Supportive Housing to promote well-being and stability. The Benefits Specialist is part of a specialized team working across four SRO sites in The Bronx and Brooklyn. The Benefits Specialist will be assigned a caseload of clients at one of those sites and will coordinate closely with case management, recreational and operations staff at the building to which they are assigned
 
East 119th Street Veterans Residence is a single room occupancy (SRO) residence that provides permanent, supportive housing to formerly homeless single veteran men and women, including those diagnosed with a mental illness. Benefits Specialist Principal Responsibilities: 
1. Conduct benefits assessments and provide individualized education, assistance, referrals and advocacy to help clients obtain and retain any and all benefits to which they are entitled, including accompanying clients to appointments as needed.
 2. Work with clients and their medical and behavioral health providers to document disability to support SSI/SSD applications and appeals.
 3. Provide individualized money management and budgeting coaching and as needed connect clients to services to adult guardianship or representative payee services.
 4. Develop and deliver workshops on topics related to benefits, budgets and money management.
 5. Utilize best practice approaches and evidence-based interventions to maximize client outcomes.
 6. Maintain service documentation to include client progress notes, observations and interactions about clients and incident reporting when necessary.
 7. Participate in internal and external case conferencing activities.
