In this meaningful, fast-paced role, you'll support youth facing mental health crises by connecting them with essential resources, providing stabilization support, and collaborating with a multidisciplinary team that includes therapists, peer support specialists, respite workers, and care team assistants. CY-FAST offers a range of services such as mobile outreach, respite care, group therapy, and coordination of Medicaid hospitalizations, all grounded in our mission to promote mental, emotional, and physical wellness through compassionate, comprehensive care for youth and their families.
Schedule: Monday&ndash;Friday, 9:00 AM&ndash;6:00 PM (hours can vary based on program's needs) 
Compensation: $21.15&ndash;$29.58 DOE and Licensure

What You'll Do as a Case Manager<ul><li>
Conduct client assessments and help develop individualized care and crisis stabilization plans for youth</li><li>
Provide support to youth experiencing mental health challenges through connection to resources and services</li><li>
Coordinate care with therapists, medical providers, and other members of the CY-FAST team</li><li>
Assist with and support services including mobile outreach, respite coordination, group therapy, and Medicaid hospitalization oversight</li><li>
Monitor client progress and adjust care plans as needed to support ongoing stabilization and success</li><li>
Maintain accurate, timely documentation and up-to-date case notes</li><li>
Collaborate closely with a multidisciplinary team to ensure comprehensive, wraparound care</li><li>
Use creativity, empathy, and strong problem-solving skills to help clients navigate challenges and build resilience</li></ul>
Qualifications
Applicants must meet ONE of the following:
Note: Applicants under the &quot;working toward SSW&quot; pathway must obtain their SSW license within 24 months of hire.<ul><li>
Bachelor's degree with Social Service Worker (SSW) license</li><li>
Bachelor's degree in any field with willingness to pursue SSW licensure</li></ul>
Additional Requirements:<ul><li>
Valid driver's license</li><li>
Basic understanding of Social Work principles</li><li>
Familiarity with social welfare policy and human development</li><li>
Ability to problem-solve and maintain a client-centered approach</li><li>
Spanish speakers may qualify for increased wages after testing</li></ul>
