Job Summary:</div>
The Wayne Health Mobile Units represent the future of healthcare, providing &lsquo;care your way, where you live and work,&rsquo; with the long-term goal of improving population health outcomes. The Clinical Lead will coordinate and oversee activities of the onsite clinic and team members to ensure compassionate, effective and efficient delivery of high-quality clinical care for all patients.<div></div><div>

project completion.</div><ul><li>Supervises, monitors and evaluates daily performance of team members to ensure duties are</li></ul><div>
completed accurately, efficiently and timely.</div><ul><li>Provides leadership and staff guidance, supports challenging situations and implements</li></ul><div>
policies and procedures.</div><ul><li>Manages the delivery of high-quality patient care and implementing best practices and</li></ul><div>
guidelines to ensure that clinical standards are maintained.</div><ul><li>Ensures high quality care for patients by handling concerns or complaints and recommend</li></ul><div>
corrective action as appropriate with guidance from the MHU Manager.</div><ul><li>Leads by example by maintaining excellent communication and effective working relationships</li></ul><div>
with patients, community partners, team members, providers and leadership.</div><ul><li>Assist with and attends meetings to relay pertinent information to team members.</li><li>Assist with training new staff on clinical procedures, protocols, policies and sign off on</li></ul><div>
competency checklists.</div><ul><li>Manages set up and breakdown of the site to ensure cleanliness, professionalism, high</li></ul><div>
visibility, and safety are met.</div><ul><li>Ensures all equipment and company vehicle remains in operating condition.</li><li>Provides clinical care to patients with compassion and respect that is timely and efficient with</li></ul><div>
maintaining and protecting patient confidently according to departmental and HIPAA</div><div>
guidelines.</div><ul><li>Documents in patients&rsquo; medical record that is accurate, factual, complete, timely, organized</li></ul><div>
and compliant with healthcare laws and Wayne Health standards.</div><ul><li>Performs and document in the electronic medical record patients&rsquo; vital signs (temperature,</li></ul><div>
pulse, blood pressure and respirations) height, weight, allergies; alert physicians of any</div><div>
abnormalities in vital signs or appearance.</div><div>
&middot; Prepares and assists physicians with physical exams for patients. &middot; Instructs and/or assists patients in specimen collection, tests and procedures. &middot; Obtains labs, point of care testing and urine while ensuring proper labeling of specimens. &middot; Administers vaccines, medications and infusions. &middot; Performs diagnostic testing such as retinal imaging. &middot; Educate and counsel patients and family members on health conditions, desired results of treatment plans and other related areas. &middot; Makes referrals to Community Health Workers (CHWs) for social services &middot; Ensures all paperwork is completed accurately, timely and efficiently. &middot; Ability to travel to Mobile Unit sites and be flexible with changes in locations/sites to external factors (i.e. weather, cancellations, add-ons, etc.) &middot; Provides clinical services to home bound patients and their caregivers in their homes &middot; Other duties as assigned.</div><div></div><div>

Job Summary:</div>
Provides biopsychosocial assessments and treatment interventions through the use of structured educational and therapeutic groups and individual treatment formats. Provides case management and care coordination to facilitate access to appropriate services and supports and adherence to treatment plans.<div></div><div>
Job Description:</div><div>
Essential Duties and Responsibilities:</div><ul><li>
Provides mental health and/or chemical dependency treatment, assessments, individual or group clinical services</li><li>
Coordinates patient referrals for substance abuse treatment, accommodations for homeless patients, Medicaid, Medicare, SSI, SSD, bus cards, shelters and work therapy as needed</li><li>
Provides accurate patient assessment; evaluates data; develops individualized Plan of Care following guidelines for Person-Centered Plans that includes participation of the patient to identify and understand patient&rsquo;s problems and circumstances. Explores available resources and services; educates and supports the patient regarding recipient rights, treatment options and services available</li><li>
Provides for a multi-disciplinary approach to treatment by coordinating with other staff and with persons identified by the patient; coordinates care with referring physician or agencies; assesses patient progress and modifies the treatment plan accordingly</li><li>
Maintains availability to patients for whom he/she is the primary clinician or arranges appropriate clinical coverage</li><li>
Maintains timely documentation in patient clinical records according to clinic policy and procedure. Ensures that documentation meets licensure, accreditation and third party payer standards</li><li>
Maintains timely contact as needed with third party payers to ensure authorization for services provided</li><li>
Networks as applicable with referral sources, courts, third party payers and other appropriate resources</li><li>
Participates in case conferences to maintain level of supervision</li><li>
Participates in quality management program, and adheres to accessibility, safety, and infection control standards</li><li>
Conforms to the ethical code of conduct of his/her professional organization, to clinic policies and procedures and to legally sound business practice</li><li>
Maintain a current professional license, professional liability insurance and credentialing by Wayne Health or DMC as applicable</li><li>
Attend required meetings</li><li>
Participate in professional development activities to keep current with health care trends and practices</li><li>
Obeys confidentiality as required by all federal, state, regulatory and Wayne Health policies</li><li>

Education, Licensure/Certification, and Training:</div><ul><li>
Master&rsquo;s degree in Social Work</li><li>
Social Work licensure to practice in State of Michigan</li><li>
Two (2) years of experience in behavioral health preferred</li></ul><div></div><div>
Skills and Abilities:</div><ul><li>
Knowledge of the principles of addiction medicine, and mental health clinical practices</li><li>
Knowledge of organization policies and procedures</li><li>
Knowledge of community resources and referral agencies</li><li>
Knowledge of requirements of third party payers</li><li>
Skill in interviewing and eliciting information and cooperation from patients</li><li>
Skill in establishing and maintaining effective working relationships with patients, staff and the public</li><li>
Ability to interpret and communicate clinical and professional rules, regulations and procedures</li><li>
Experience and training with various patient populations and modes of service delivery in both mental health and those relating to chemical dependency treatment preferred</li></ul><div></div><div>
