<div>Description:

 The Service Coordinator plays an essential role in supporting individuals and families served by WCDC, Inc. This position ensures that clients have access to coordinated, person-centered, and culturally responsive services that promote stability, independence, and well-being.
DUTIES/RESPONSIBILITIES:<ul><li>
Ensure compliance with safety &amp; person-centered support plans when executing programming needs of the clients.</li><li>
Conduct intake and assessments to determine client needs, strengths, and goals. Update assessments as appropriate.</li><li>
Develop individualized service or care plans and support clients in achieving their goals.</li><li>
Coordinate services and referrals with community partners, agencies, and internal programs.</li><li>
Provide ongoing follow-up, advocacy, and support to ensure successful connection to resources.</li><li>
Maintain accurate, timely, and confidential client documentation in accordance with agency and funding requirements.</li><li>
Work collaboratively with internal and external team members to promote communication, service integration, and positive outcomes.</li><li>
Crisis intervention and handling of client disciplinary issues and behavior modification for inappropriate behaviors.</li><li>
Support programming and instruction in the following areas:

<li>Daily Living Skills</li>
<li>Behavioral Skills</li>
<li>Whole Person Health</li>

</ul></li><li>Attend regular team, client advisory, client staffings &amp; internal department meetings.</li><li>
Participate in company committees and attend training and seminars to continue developing professional skills.</li><li>
Maintain professional development and continuing education requirements as applicable to role and licensure.</li><li>
Represent WCDC, Inc. and uphold its mission, vision, and values in all community and client interactions.</li><li>
Maintain a positive working relationship with other personnel, volunteers and consumers.</li><li>
Notify the Director of Day Programming of client complaints regarding the program or staff and act upon complaints per grievance policy and procedures.</li><li>
Ensure timely completion of internal and external incident reporting including dependent adult and child abuse reporting.</li><li>
Complete Client Satisfaction Surveys annually with clients and/or Guardians.</li><li>
Provide activity and floor coverage for breaks and staff absences when appropriate.</li><li>
Supervise social work students in fulfilling service coordinator experience practicum requirements. Provide feedback to student practicum supervisors when requested.</li><li>
Assist President &amp; management team with identifying opportunities for service expansion, including but not limited to residential and hourly supported community living (SCL), adult day habilitation and supported employment programming.</li></ul>

Requirements:

 REQUIRED SKILLS/ABILITIES:<ul><li>
Occasional after-hours or weekend work may be required to attend programming.</li><li>
Strong interpersonal and communication skills with a client-centered, trauma-informed approach.</li><li>
Ability to establish and maintain effective relationships with clients, co-workers, and community partners.</li><li>
Excellent organizational and time management skills, with attention to detail and accountability for follow-through.</li><li>
Commitment to diversity, equity, inclusion, and cultural humility.</li><li>
Familiarity with client and personnel record-keeping systems (e.g., SetWorks, Paylocity).</li><li>
Valid driver&rsquo;s license is required.</li></ul>
EDUCATION AND EXPERIENCE:<ul><li>
High School Diploma or equivalent required.</li><li>
Associate or bachelor&rsquo;s degree in social work, human services, psychology, or related field is preferred.</li><li>
Experience working in a direct support role is preferred.</li><li>
Experience working in a nonprofit or mission-driven organization is preferred.</li><li>
Familiarity with local community resources, social service systems, and public benefits is preferred.</li></ul>
PHYSICAL REQUIREMENTS:<ul><li>
Standard office environment with controlled temperature and moderate noise level.</li><li>
Prolonged periods of sitting at a desk and working on a computer.</li><li>
Frequent use of hands and fingers to operate keyboards, office equipment, and telephones.</li><li>
Regular reaching with hands and arms, and occasional bending or stooping.</li><li>
Ability to communicate clearly in person and over the phone.</li><li>
Occasionally standing, walking, or moving within the office environment.</li><li>
May be required to lift or move equipment weighing up to 25 pounds.</li><li>
Occasional travel to off-site locations, meetings, or events may be required.</li></ul>
Work hours are typically M-F 8 am - 4 pm
